Andrews, Special Date: January 27, 2014 Re: Review of Execution Process for the Execution of Inmate McGuire A30S-382 Conducted on January 16, 2014 As directed, I have evaluated the activities of DRC staff related to the execution of inmate Dennis McGuire A305-382 carried out on January 16, 2014. This memo serves as my report to you as to my findings after observation and reviewing the documentation of the execution and pre and postexecution activities and procedure. I previously reviewed DRC policy 01-COM-ll Rev. October 10,2013. COMPLIANCE My review of the execution activities of the DRC staff finds no issues of non-compliance with DRC Execution Policy 01-COM-ll Rev. October 10, 2013. At least three Medical Team members, two of whom were authorized to administer drugs under Ohio Law were available and used in conducting the execution. The drugs required by policy were used. The medical and mental health staff conducted assessments of the inmate at the appropriate times; they reviewed the inmate's medical and mental health charts and documented the reviews. All functions required to be performed by medicallyqualified persons as described in 01-COM-ll were performed by medical team members. All Execution Team members were appropriately trained and qualified to perform their roles on the team. Finally, there were no variations from procedures as stated in policy. OBSERVATIONS And RECOMMENDATIONS I have no observations or recommendations. FUTURE POLICY CONSIDERATIONS I find no reason for revision of policy for future executions Summary I have observed the process and reviewed all associated documentation and found that the process was followed and all documentation was in order and in compliance with 01-COM-ll rev. October 10, 2013.
