'l In" E.- a a INTER-OFFICE MEMORANDUM .1 F: To: Sgt. R. Bailey Date: September 25, 2009 From: Sgt. Gary Clark On Wednesday, September 16, 2009, I Sgt. G. Clark #1619 was assisting the Disciplinary Board on several inmates? hearings while assigned to 6South. Inmate Worthen, Trevis #0929227 case was reviewed. Inmate Worthen was infonned of the rule violations and asked how he pleads to each charge. Inmate Worthen stated, was wrong, I was wrong?. The infractions days were calculated and given to inmate Worthen. Inmate Worthen was given 30 days in disciplinary lockdown. Inmate Worthen seemed confused and was asked did he understand his sentence. Inmate Worthen continued to repeat he was wrong but added what about the deputy. Inmate Worthen was asked to sign his sentence paperwork but refused stating again he was wrong but what about the deputy.When asked about the deputy inmate Worthen never responded. Inmate Worthen was escorted back to his cell where a few minutes later he was escorted to zone 700 by D.O.C. Harper. Of?cer Harper informed me inmate Worthen attempted to hang himself in his cell and she was informed by his cellmate. I spoke with inmate Worthen concerning the following suicide attempt. Inmate Worthen stated, ?that the only thing he did wrong was cuss the deputy out, but he did not do anything else for the deputy to beat him and choke him while he was handcuffed.? I informed inmate Worthen he did not give us anything to work with when he stated he was wrong. I also informed inmate Worthen I would investigate the matter further but I needed him to go to medical with Of?cer Harper so mental health could speak with him concerning the suicide attempt. Later I informed Capt. M. McKee I had concerns with a couple of cases and that I would like to further investigate them by watching the video, Capt. McKee agreed. On Thursday, September 17, 2009 I proceeded to security to view the video on inmate Worthen. I observed an inmate being escorted into zone 700; the inmate sat on the bench and looked up at the of?cer. I observed the inmate being grabbed around the neck area pushed against the back of the bench and later to the floor. I informed Sgt. Cockhren that a copy of the incident will be required for Capt. McKee in disciplinary. I proceeded to the Hope Center to inform Capt. McKee and DO Ross-Hamilton of my ?ndings, neither one was there so I informed Lt. V. White there was no evidence of major or serious rule violations found on the video and the inmate needed to be relocated back to general p0pulation. I changed the sentence sheet from 30 days to 3 days after speaking with Of?cer Ross-Hamilton over the phone and turned it over to Sgt. A. Williams in classi?cation. On Monday, September 21, 2009 I informed Capt. McKee of my ?ndings and she informed me she would need a copy of the video from security. On Tuesday, September 22, 2009 upon my return from days off I proceeded to security to get a copy of the Video from gt. Cockhren. Capt. McKee was not present on this day so I turned the copy over to Lt. V. White.
