On or about 07/22/14, Jacob Zimmerman did possess a ?rearm. This behavior is in violation of rules# 1 and 13 his Rules of Community Supervision, which were signed by Mr. Zimmerman on 07/02/ 14. 2. On or about 07/22/14, Jacob Zimmerman did consume alcohol, this is in violation of the conditions of his bond. This behavior is in violation of rules# 1 his Rules of Community Supervision, which were signed by Mr. Zimmerman on 07/02/ 14. DESCRIPTION (Substantiating details are to be a brief chronological narrative and include summary "statement. impact on victim. loss of propertyor other results of violation.) According to Adams County Sheriff Department Incident Report 14-4525, on 7/22/14, Deputy Jacob Nielsen was dispatched to a report of a male lying in the roadway on Deerborn Avenue in the Township of Adams, Wisconsin. Deputy Nielsen observed an adult male, later identified him as Jacob Zimmerman, lying in the roadway on his back. There was a bicycle next to him and a black Smith and Wesson .40 caliber handgun sticking out of the bottom of his shorts (Allegation Deputy Nielsen conducted a pat down search of Zimmerman for the of?cer?s safety and to check for injuries. Zimmwman woke up during the search but was not lucid. He said he Was sleeping but it did not appear he knew his whereabouts. Deputy Nielsen observed an odor of intoxicants coming ??om Zimmerman?s breath and his speech was slurred. Jacob refused to submit a? PET (Allegation Zimmerman had no idea he was in poSSession of a ?rearm. Deputy Nielsen obtained information that Zimmerman was a convicted felon, on probation and was on bond in 2 separate cases. Deputy Nielsen had requested ambulance and when it arrived Zimmerman refused medical treatment. Zimmerman was placed in custody and transported to Adams County Jail and charged with Possession of Firearm and Bail Jumping, On 07/23/14 Zimmerman gave a signed statement to Agent Emily Umbreit. He admits having consumed 2 cans of Mike? Hard Lemonade at Kwik Trip in Adams. He did not recall too many details of the evening and believes he was drugged by someone. He denied having knowledge of how he came to possess a handgun. On 07/23/ 14 Agent Emily Umbreit and CFS Vonda Benson staffed Zimmerman?s violations and a decision to revoke was made. 'On the Compas Core Assessment, Zimmerman?s overall violent recidivism risk potential is low with a score of 2 and general recidivism risk potential is medium with a score of 5. According to the criminogenic needs scale, Zimmennan?s scores included highly probable in the areas of substance abuse and criminal associates/peers. His case plan focused on her substance abuse needs. On 07/23/ 14 Agent Emily Umbreit and CPS Vonda Benson staffed Zimmerman?s violations and potential Zimmerman had approximately 5 pending cases along with his current violations. Outpatient treatment, formal and informal warnings and amending rules were discussed and found to be inappropriate due to the seriousness of his violatioos. Agent Umbreit looked into VA AODA-Impatient programs which Zimmerman would be eligible to participate in. The VA had only been able to offer AODA outpatient programing. This was also staffed with CPS Vonda Benson and found to be inappropriate due to his recent behavior and the seriousness of his violations.
