After the compliance date, if the violations listed below remain uncorrected; an enforcement action may be initiated to obtain the correction of these violations. This action could include civil penalties. 1. 10. The urinal on the left (men's central restroom) was leaking wastewater from the drain trap, creating an unsanitary condition with wastewater running across the floor. This is a violation of 41 0 IAC 6?9- The 3rd toilet from the left in the women?s restroom was spraying water extensively from the supply line, creating a wet, unsanitary condition. This is a violation of 41 0 Two of the women?s four toilets were not functioning properly. The one on the right was completely inoperable and the 3'd from the left would not flush with adequate force to be useable. This is a violation of 41 0 IAC This leaves only two functioning women's toilets in the camp. The women's restroom did not have enough functioning lights to provide at least ten (10) foot- candles of illumination throughout the room. Half of the bulbs had failed. This is a violation of 410 IAC The men?s sink on the left was leaking water from the cold water supply valve, creating an unsanitary condition in the restroom. This is a violation of 410 IAC The women?s restroom LEFT outlet was not protected by a functioning ground fault circuit interrupter. This is a violation of 410 Shelter number 19 (uninhabited) was not secured to prevent access (door unlocked), Vacant shelters 16 and 18 also have unsecure, missing windows. This is a violation of410 A tire off the rim, several pots, and a bucket of stagnant water which can all provide breeding sites for mosquitoes were located near unit 20 (now inhabited). This is a violation of 41 0 The outdoor circuit breaker box (central power distribution) behind unit 12 was not maintained in a safe condition because the face plate covering the live electrical components was not adequately secured with a screw. This is a violation of 410 Putrescent food scrapsiwastes have been discarded behind the front mobile home near the steps, as if to feed animals. A used stove between unit 10 and the back mobile home has not been removed from the camp. This is a violation of410 IAC 12. The men's restroom door was propped open, allowing insects and animals access. This is a violation of 41 0 IAC Also note that an air compressor was being operated continuously within the men's restroom to supply compressed air to the adjacent construction site. The loud, continuous noise from this would be a nuisance to anyone attempting to utilize the sanitary facilities. The construction of a new camp, the conversion of premises to a camp, or enlargement of the existing camp requires written notice to the Environmental Public Health Division. This notice must be given not later than 60 days prior to starting construction, enlargement, or conversion. Plan submittal may be required by the Division. 410 L40 6?9-2.
