The following violations of IC 16-41?26 and/or 410 IAC 6?9 were observed and must be corrected prior to
opening and licensing.

1.

The fire extinguisher located at the east end was discharged and not properly recharged or tested.
This is a violation of 410 IAC 

The smoke detector was not working. This is a violation of 410 IAC 
An odor of gas (mercaptans) was evident in the kitchen. This is a violation of410 IAC 

There were missing light bulbs in wall and ceiling fixtures. This is a violation of 410 IAC 6?9?


The east bathroom sink was not operable. This is a violation of 410 IAC 

Beds used for double occupancy were provided in single sex accommodations. This is a violation of
410 

The construction of a new camp, the conversion of premises to a camp, or enlargement of the existing
camp requires written notice to the Environmental Public Health Division. This notice must be given not
later than 60 days prior to starting construction, eniargement, or conversion. Plan submitta! may be
required by the Division. 410 IAC 6-9-2.
