Contact Mr. Rob DeBeck when these violations have been corrected. After the compliance date, if the violations listed below remain uncorrected; an enforcement action may be initiated to obtain the correction of these violations. This action could include civil penalties. 1. Containers that create breeding grounds for mosquitoes were observed at the frost free hydrant area (water trough), restroom (garbage container), south of the vacant mobile home lot (tires), laundry (barrel?buckets), and 2 (garbage container). This is a violation of 41 0 IAC 2. Wastewater was backing up onto the floor through the floor drain in the laundry. This is a violation of 410 IAC 3. The vacant mobile home at the north was not secured to prevent unauthorized entry. This is a violation of410 4. The doors on shelters numbered 2 and 3 and the restroom doors did not fit in their frames. This is a violation of 410 5. The restroom window did not have a screen. This is a violation of 41 0 6. A cat was running at large at the garbage dumpster area. This is a violation of 41 0 IAC The construction of a new camp, the conversion of premises to a camp, or enlargement of the existing camp requires written notice to the Environmental Public Health Division. This notice must be given not later than 60 days prior to starting construction, enlargement, or conversion. Plan submittal may be required by the Division. 410 IAC 6-9-2. Robert L.
