Roaches were observed in the bathrooms. This is a violation of 410 IAC 2. Non mixing type faucets were installed on bathing and hand washing facilities. This is a violation of 410 3. The wooden partitions around the toilets were not constructed of smooth, nonabsorbent, easin cleanable material (the plywood was rotting). This is a violation of410 4. The rest room was not maintained in a clean and sanitary condition. This is a violation of 410 6? 5. There was a broken toilet seat in the women's bathroom. This is a violation of 410 IAC 6. All living units in the camp were not assigned a number. This is a violation of 410 7. The maximum number of residents allowed in each shelter was not posted near the primary entrance to the shelters. This is a violation of 410 IAC 8. The unused shelters in the eastern half of the row housing did not have all broken doors and windows secured closed. This is a violation of 410 9. An excess of cats (15 cats kittens were observed in one location) creates a situation which may transmit disease. This is a violation of410 The construction of a new camp, the conversion of premises to a camp, or enlargement of the existing camp requires written notice to the Environmental Public Health Division. This notice must be given not later than 60 days prior to starting construction, enlargement, or conversion. Plan submittal may be required by the Division. 410 MC 6?9~2. Kenneth J.
