Dear Mayor Blucher.

The purpose of this letter is to place the City of North Port on notice of the claim
of Danielle Lauren Drake against the city pursuant to F.S. ?768.28. Ms Drake's claim
pertains to the injuries she sustained on the above date and location.

On that date and at that location, Ms Drake was severely iniured by the
excessive use of force by North Port Police Department K-9 officer Keith Bush and K--Q
Tomy,

The following information is being provided pursuant to ES.
