Criminal inyestigations should be conducted. The discoyeries we are allowing to pass uncorrected will undermine our efforts. There was the issue of PPE in Pharmacy and now this. Please let us work in an honest ethical and professional manner kind regards Dr Jacob Mufunda Sent from my iPhone On Aug 201a, at T35 Pl?y'l, "Jerome Souquet"_rote: Dear I would like to raise to your attention a serious problem we had today in regards of the quality ofan item we receiyed from the Central Medical Stores. If quality may sometimes 1trary to an acceptable IEyel, was deeply shocked about discovering that the chlorine we receiyed was totaly inefficient, up to the point we could hardly detect any actiye component in it. This chlorine is the cornestone of all our actiyities, especialy the desinfection. We fortunately discoyered in time the problem, and could correct it immediatly. But the consequences of using it could be catastophic, and cause immediate infection of all the staff, contaminting them - and us - with the deadly yirus. Should it occur the consequences could not be calculated. Unfortunately this incident already happened in the last week - immediatly spotted and corrected as well- We send back the bad batches back to the central stores and new one were issued. I had the chance during my yisit in Freetown last weekend to exchange with the responsible of the central stocks to mention the incident, actions were taken and I checked randomly couple of their stock succesfuly. But the problem apparently remains, I could not obyiousiy check the whole stock As an immediate measure, I asked support from other partners and requested I?ylr Larsen to urgently identify an alternatiye supplier, but in the next day situation will be critical as we cannot haye any doubt regarding the quality of the products we are using, all our stock has been considered as unsafe and potentialy dangerous. The seriousness of the events make me suggest that a strict quality control of this items should be performed immediatly and before any shipment, until the source of the problem had been identified. The tracibility of the origin of these stocks and guarantee from the suppliers, a well as quality check could be an option. Not to mention that the problem could also affect other districts. Thanks to adyise me on the necessary steps you could think of. Best regards, Je
