The application indicates that this request is for a new. year-round program. operated by National Neuro. Inc.. to serve grades 2 for both males and females. with a prOposed start date of September I. 20l 5. The following is a list of some of the signi?cant discrepancies identi?ed during the preliminary review of the application and supporting materials: a The cover letter states the proposed licensee is National Neuro. Inc.. as does the letter from the Secretary of State. However. the application states that Green Mountain Academy is applying for program approval. The Bureau is unclear whether National Neuro is doing business as Green Mountain Academy. or it Green Mountain Academy is working in conjunction with National Neuro. Inc. for this program approval. based on this statement. Green Mountain Academy refers to a residential component of their program throughout the application materials submitted. There is no evidence of a New Hampshire Department of Health and Human Services certi?cate of residential approval. The materials submitted include health. tire. and life safety inspection documents issued to Lakeview: (D.B.A.) Green Mountain Academy. The Bureau is unsure why the inspection documents are not issued to Nation Neuro. Inc., as National Neuro. Inc. is listed as the entity applying for program approval. The cover letter states ?The proposed name of the school to be owned and Operated by National Neuro. Inc.. is Green Mountain Academy. (Please note that "Green Mountain Academy" is not and will not be a stand-alone corporation. Rather, upon approval of our application, we intend to obtain consent from the Bureau of School Approval Facility Management to apply for National Neuro, Inc.'s use of the trade name "Green Mountain Academy"). The Bureau is unaware of a School Safety 8. Facility Management Bureau and how that applies to Green Mountain Academy's use of the Green Mountain Academy name. The insurance documents provided are issued to Lakeview Management, Inc. The Bureau is unable to accept documents that are not issued to the entity that is applying for program approval. Financial Statements provided are issued to Lakeview Neurorehabilitation Center. Inc. The Bureau is unable to accept documents that are not in the name of the entity that is applying for program approval. The Professional Development Plan is dated 812/] and lists Lakeview School. The Bureau is unable to accept a Professional Development plan that does not list the name of the entity that is applying for program approval. The curriculum is lacking General Music. Art. Space Science. or Health 8. Wellness courses. The Bureau is not able to accept curriculum that does not include all areas of study required to meet the standards for program approval. The NHDOE is unable to approve Green Mountain Academy as a Special Education program for grades 9-12. based on the submitted application and supporting materials as the materials lack the necessary requirements to meet the standards for program approval. The Bureau recommends that Green Mountain Academy review all submitted application materials for consistency and clarity in an effort to correct any and all discrepancies prior to the resubmission of the application. Upon receipt of a completed application and materials. the Bureau will perform another review and determine approval status.
