BM093419 of Philadelphia, Philadelphia County, was suspended pursuant to the Order of the Court of Common Pleas of Philadelphia County dated Jan. 20, 2009, which the court issued under section 4355 of the Domestic Relations Code. CO260391 of Philadelphia, Philadelphia County, was ordered to pay a civil penalty of $1,000 because she violated the law in that she maintained an unlicensed salon, and in that she aided and abetted an individual in the unlicensed practice of cosmetology. CY193568 of Greensburg, Westmoreland County, was ordered to pay a civil penalty in the amount of $1,000 because it practiced cosmetology in a grossly incompetent and/or unethical manner by possessing methyl methacrylate. CO261959 and CL179427 of Perry Hall, Md., had her license revoked, and she was ordered to immediately cease and desist the practice of the profession because she violated the law in that she procured a license to practice as a cosmetologist by dishonest or unethical means. CL180027 of Baltimore, Md., had her license revoked and was ordered to immediately cease and desist the practice of the profession because she violated the law in that she procured a license to practice as a nail technician by dishonest or unethical means. CO259792 of Egg Harbor, N.J., had his license permanently revoked, and he was ordered to immediately cease and desist the practice of the profession,, because he violated the law in that he procured a license to practice as a cosmetologist by dishonest or unethical means. DS036781 of Marlboro, N.J., was suspended for a period of one year, based upon him being found guilty of health care claims fraud, a crime or misdemeanor involving moral turpitude. DS030424L of Severna Park, Md., had his license indefinitely suspended to practice dentistry in the state of Maryland for improper prescribing practices. MD044780L of Yardley, Bucks County, was ordered to pay a civil penalty of $5,000 and a public reprimand was placed on his permanent board record because he was convicted of a felony in a federal court. MA051320 of Wilkes-Barre, Luzerne County, had his physician assistant license suspended for two years, retroactive to Oct. 28, 2008, based on findings that he is unable to practice with reasonable skill and safety to patients by reason of mental or physical illness or condition, or physiological or psychological dependence upon alcohol, hallucinogenic or narcotic drugs or other drugs, which tend to impair judgment or coordination. RT003905 of Houston, Washington County, was assessed a civil penalty of $250 because he practiced as an athletic trainer without holding a valid certificate during the period May 2005 through Jan. 25, 2006, and used the abbreviation "ATC" after his name on his cover letter, without holding a valid athletic trainer certificate. (02/24/09) York County Ashley Lee Black, a.k.a, Ashley Lee Coutant, license no. RT003636 of Stewartstown, York County, was assessed a civil penalty of $250 because she practiced as an athletic trainer when she was not currently licensed and registered under the Medical Practice Act. (02/24/09) Out-of-State Frederick Ralph Miranda, M.D., license no. MD041452L of Salinas, Calif., had his Pennsylvania license indefinitely suspended until such time as he provides documentary evidence to the board that his California license is reinstated to unrestricted status, based on disciplinary action taken against his license to practice medicine by the proper licensing authority of California and his having failed to report this disciplinary action to the board within 60 days. MD053159L of Portland, Ore., agreed to the permanent voluntary surrender of his license to practice medicine in Pennsylvania because he had disciplinary action taken by a proper licensing authority of another state against his license to practice the profession. MD050374L of Hollywood, Fla., was ordered to pay a civil penalty of $2,500 because he had disciplinary action taken by the proper licensing authority of another state against his license to practice the profession. RN513611L of Pittsburgh, Allegheny County, had her license reinstated to probationary status after providing proof that her criminal probation has been completed and has completed an evaluation by an approved treatment provider, who certified that she can practice nursing with reasonable skill and safety to patients. RN539813 of Glassport, Allegheny County, was indefinitely suspended for no less than four years, retroactive to April 14, 2008, and after one year of active suspension, upon petition, such suspension may be stayed in favor of no less than three years of probation, on the grounds that he is unable to practice the profession with reasonable skill and safety to patients by reason of mental or physical illness or condition, or physiological or psychological dependence upon alcohol, hallucinogenic or narcotic drugs or other drugs, which tend to impair judgment or coordination. PN257607L of Ford City, Armstrong County, was indefinitely suspended for no less than one year, with such suspension immediately stayed in favor of no less than one year of probation, subject to the board's terms and conditions because he failed to conform to the accepted standard of care in the practice of licensed practical nursing when he administered intravenous push medications to a patient when he was not authorized to do so. RN349505L of Altoona, Blair County, was ordered to pay a civil penalty of $1,000 and his license was suspended; however, said suspension was immediately stayed in favor of probation subject to terms and conditions set forth in his agreement. RN181807L of Bristol, Bucks County, was indefinitely suspended for no less than three years, with such suspension immediately stayed in favor of no less than three years of probation, subject to the board's terms and conditions, because she is unable to practice professional nursing with reasonable skill and safety to patients by reason of mental or physical illness or condition, or physiological or psychological dependence upon alcohol. PN264169 of Westover, Clearfield County, was indefinitely suspended because she was convicted or pled guilty or entered a plea of nolo contendere or was found guilty by a judge or jury of a felony crime or a crime of moral turpitude in the courts of this commonwealth, the United States, or any other state, territory or country, or received probation without verdict, disposition in lieu of trial or an accelerated rehabilitative disposition in the disposition of felony charges, or was dishonorably discharged or was discharged under circumstances amounting to dishonorable discharge from the military forces of the United States or of any other country. PN250632L of Grantville, Dauphin County, had her practical nursing license reinstated, subject to a three-year period of probation and monitoring by the Professional Health Monitoring Program's disciplinary monitoring unit, based on findings that she has met the requirements for reinstatement to probationary status. RN317601L of Erie, Erie County, had her nurse certificate suspended for no less than three years, retroactive to Oct. 28, 2008, based on findings that she violated the terms of the Voluntary Recovery Program Consent Agreement. RN525793L of Belle Vernon, Fayette County, had her license suspended for a period of three years, stayed in favor of three years probation subject to terms and conditions, because she violated the law in that she is unable to practice the profession with reasonable skill and safety to patients by reason of mental or physical illness or condition, or physiological or psychological dependence upon alcohol, hallucinogenic or narcotic drugs or other drugs, which tend to impair judgment or coordination. PN004734Y of Homer City, Indiana County, was required to pay a civil penalty of $1,000 and must cease and desist from engaging in, or offering to engage in, the profession of nursing in Pennsylvania, because he practiced without having a valid, unexpired, unrevoked and unsuspended license and used “LPN” letters intending to imply that he was a licensed practical nurse. PN265850 of Old Forge, Lackawanna County, had her practical nursing license suspended for two years, after which said license will be placed on probation for three years, based upon her felony conviction under the Drug Act. RN562039 of Ephrata, Lancaster County, was indefinitely suspended for no less than three years, with such suspension immediately stayed in favor of no less than three years of probation, because she was unable to practice the profession with reasonable skill and safety to patients by reason of mental or physical illness or condition, or physiological or psychological dependence upon alcohol, hallucinogenic or narcotic drugs or other drugs, which tend to impair judgment or coordination. RN196661L of Willow Street, Lancaster County, was indefinitely suspended on the grounds that she is unable to practice the profession with reasonable skill and safety to patients by reason of mental or physical illness or condition, or physiological dependence upon alcohol, hallucinogenic or narcotic drugs or other drugs, which tend to impair judgment or coordination. PN261515L of Lititz, Lancaster County, had her nursing license suspended for no less than three years, retroactive to Sept. 9, 2008, based on findings that she is unable to practice nursing with reasonable skill and safety to patients by reason of addiction to alcohol or other drugs which tend to impair judgment or coordination. RN203799L of Palmyra, Lebanon County, had her nursing license suspended for no less than three years, retroactive to Dec. 8, 2008, based on findings that she is unable to practice nursing with reasonable skill and safety to patients by reason of addiction to alcohol or other drugs which tend to impair judgment or coordination. PN061493L of Bethlehem, Lehigh County, had her nursing license actively suspended for three months, then placed on probation for at least one year, based on her guilty pleas to offenses of moral turpitude. (01/28/09) Luzerne County Denise M. Hackenberg, LPN, license no. PN262077 of Hazleton, Luzerne County, was indefinitely suspended for no less than three years, with such suspension immediately stayed in favor of no less than three years of probation, subject to the board’s terms and conditions, because she is addicted to Page 10 of 17 hallucinogenic or narcotic drugs, which tend to impair judgment or coordination, and is unfit or incompetent by reason of negligence, habits or other causes. RN515520L of Conshohocken, Montgomery County, was suspended for at least three years, stayed in favor of a three-year probation, based on his violation of the terms and conditions of the voluntary recovery program consent agreement and order and his successful completion of a boardapproved reactivation program. RN516166L of Villanova, Montgomery County, had her nurse certificate suspended for at least three years, retroactive to Jan. 15, 2008, said suspension immediately stayed in favor of at least three additional years of probation based on findings that she failed to fully and completely comply and cooperate with the Professional Health Monitoring Program in the monitoring of her impairment under the voluntary recovery program agreement, and she lacks an understanding of her obligations as a nurse. PN250614L of Philadelphia, Philadelphia County, was indefinitely suspended for at least three years, with the suspension stayed in favor of no less than three years of probation, on the grounds that she is unable to practice nursing with reasonable skill and safety to patients by reason of illness, addiction to drugs or alcohol, or mental impairment, and pleaded guilty to a crime. RN174625L of Irvine, Warren County, had her registered nurse license reinstated to probationary status for no less than three years, based upon her compliance with the requirements of a previously issued consent agreement. RN184245L of Monongahela, Washington County, was indefinitely suspended for no less than three years, with such suspension immediately stayed in favor of no less than three years probation, because she was unable to practice the profession with reasonable skill and safety to patients by reason of mental or physical illness or condition, or physiological or psychological dependence upon alcohol, hallucinogenic, narcotic or other drugs, which tend to impair judgment or coordination. PN099670L of Seven Valleys, York County, was suspended for no less than three years, with such suspension immediately stayed in favor of no less than three years of probation, said probation subject to certain terms and conditions, because she is addicted to alcohol, or is addicted to hallucinogenic or narcotic drugs or other drugs, which tend to impair judgment or coordination, or has become mentally incompetent.
