Dear Mr.-

The Department began its investigation ot'your water supply well located at the above -
address ("Water Supply") in response to a complaint received on October 05, 20l5 regard 
concerns with the quality ofyour water supply. Based on information obtained to date, fit) lift
investigation is necessary by the Department to determine whether the Water Supply hm'w
affected as a result oFoil and gas activities including but not limited to the drilling, altorafl

On October 7. ZOIS. the Department collected samples from your water supply. The 5 mm -
were submitted to the Department's laboratory in Harrisburg for analysis Please see the'
enclosed tables for the results ofsamples collected from your water supply by the Dep
with comparison to applicable standards. as well as documents to assist you with interp I

sample results.

The sample results showed turbidity exceeded its primary maximum contaminant level 
of NTU at a concentration of3.52 NTUs. However. this standard is only applicablito I,
unfiltered surface water sources. lron was at the secondary maximum contaminant
(SMCL) ofOJ mg/L with a concentration of0t3 trig/L. Secondary MCLs reflect I
ofthe water (Le, taster smellt etc.).
