The Court on this day sentences the defendant to probation for a period of

5 years. While on probation, he (she) shall observe the following
conditions Of probation and any others which the Court may impose at a
later date, and he (she) shall also follow the instructions of the probation
officer as to the way in which these conditions are to be carried out.

CONDITIONS

1. Report to a probation officer as directed by the Court or the probation
officer and permit the probation officer to visit him (her) at his (her)
place of abode or elsewhere.

2. Remain within the jurisdiction of the Court, unless granted permission in
writing by the Court or the probation officer to leave the jurisdiction.

3. Answer all reasonable inquiries by the probation officer and 
notify the probation officer'of any change in address or employment.

4. Avoid injurious or vicious habits.

5. Refrain from frequenting unlawful or disreputable places or consorting
with disreputable persons.

Work faithfully at suitable employment or faithfully pursue a course of
study or vocational training that will equip him (her) for suitable
employment.

7. Abstain from the_sale or use of dangerous drugs.

8. Submit to a evaluation if requested, and participate in
or narcotic treatment if deemed necessary by the Court or
the Probation Department.

9. The defendant is directed to pay to the Probation Department:
