1) rams Stanley, the plaintiff, (hereinafter "Father") is granteTrtempDrary sole
physical custody ofhis deughte-- (hereinafter 

effective immediately;

2) Mirand the Defendant, (hereinafter "Mother") shall forthwith,
within 48 hours, return to Massachusetts and deliver her into the custody of
Father at her own cost;

3) Father shall seek and obtain counseling for - forthwith and shall arrange
and have a full medical check up completed for- within 14 days.

4) Father shall produce proof compliance with Paragraph 3 to-'s attorney
in writing, Lisa Greenberg, Esqr, within 30 days,

5) Mother shall enter mental health/parenting counseling, and have completed
four sessions before she shall be permitted supervised visitation with

6) Mather shall produce written proof of compliance to the Father and -'s
attorney, Lisa Greenherg, Esq., before her supervised visits are to be scheduled.

7) Jennifer Carter, or someone referred by Jennifer Carter, Shall be the

supervisor of Mother's visits.

8) Mother shall pay the mo olthe costs of the supervlsor's time, up front in
cash, money order or bank check, before each visit.

at itthe mother complies with paragraphs 2, a, and 7 she shall be permitted
supervised visits for up to four hours, at the supervisor's convenience, every other

Saturdays

 



11) if the Mother rails to comply with paragraph 2, the Father may seek police
assistance in securing-'5 return and Mather shall he for any and all
costs incurred bythe Father, including his arid-'5 air tare and travel costs, and
Mother shall pay same to the Father within ten (10) days.

12) Ifthe Mother rails to comply with paragraph 2, the Father may seek further
emergency court orders without further notice to the Mother

13) Child support obligations to the Mother from the Father are suspended.

14) All other court orders not modified herein remain in full force and effect.
