REQUIRED CORRECTIONS AND DEADLINES:

The facility took immediate action to correct the issue of the failure of Ms. Myers not notifying a parent
immediately of accidents and injuries. No further action is needed to correct this violation. The main
concern of the Section for Child Care Regulation is that the injury occurred due to the fact that Ms. Myers
was caring for more children than the staff/child ratio submitted in the facility?s Notice of Parental
Responsibility. Due to the serious nature of this report, the facility must either submit a Notice of
Parental Responsibility with the staf?child ratio that will meet the violation (one staff member for every
16 [sixteen] three to four [3 4] year old children) or submit a plan showing how this type of ratio issue
will not occur in the future. If the facility changes the staff/child ratio in the Notice of Parental
Responsibility, each parent must receive an updated copy and sign an updated copy for the child?s record.
The plan or the updated Notice of Parental Responsibility must be on ?le with the Section for Child Care
Regulation within ten (10) days of receipt of this report.
