Allegations of StatutefRuIe Violation:
On October 20, 2010, Section for Child Care Regulation received a report alleging that a two

year old child broke her collar bone at 9:30 am on October 19, 2010 after being shoved off a bike
by another child enrolled at the facility. The parents were not noti?ed of this incident until 2:30
pm. When the child?s mother arrived at the facility, she immediately noticed that the child was
not walking right and that her shoulders were uneven. Mom immediately took the child to the

emergency 1'0 om.

Summer}I of Investigation:
On October 22, 2010, I completed an interview with Child A and Child A?s father at Phelps

County Children?s Division conference room. Robin Porter was also present. Child A provided
the following information: 

put me in it?. Child A was referring to the brace on her shoulders and across her back.
-?Child A stated that another child pushed her off a bike at daycare.

got hurt?.

have a boo-boo?. Child A pointed her left front shoulder.

?Child A stated that her collar bone just broke.

--Child A stated that she cried when she fell off the bike.

?Child A stated that she cried for a long time.

--Child A was asked if she ate lunch. Child A stated yes. Child A was then asked if she cried at

lunch. Child A stated yes.
--Child A was asked if she took a nap. Child A stated yes. Child A was asked if she cried during
