Kids Day Out and Awesome Adventures Preschool SUMMARY OF INVESTIGATION: CCFS Utter made an unannounced inspection at the facility on January 17, 2012. CCFS met with Todd Taylor, Director of Children?s Ministries. Mr. Taylor made the following statements in regard to the allegations: Note: The incident occurred on January 6, 20l2. All staff involved in the incident completed written statements. Mr. Taylor provided the CCF with copies of the written statements. The parents may begin dropping children off for preschool as early as 7:30 am. The children stay in the commons area with a staff person until enough children are present to begin splitting the children into groups. Child A was in the gymnasium with staff person, Jennifer Myers. Child A is 5 years of age. Ms. Myers was caring for sixteen children as a staff person had called in sick. Child A was riding in a Little Tykes Cozy Coupe car when the car turned over. Child A?s head hit on the ?oor. The ?oor in the is carpet over concrete. Child A began crying. Ms. Myers looked Child A over and told Child A that everything was ?ne and to stop crying. Child A went back to playing in the gym. Child A began crying when it was time to get in line to go to class. Child A?s regular teacher is Ashlee Pahlow. Ms. Pahlow noticed that Child A was acting out of character; Child A was lethargic and sleepy. Child A complained to Ms. Pahlow that Child A?s head was hurting. Ms. Pahlow asked the other children if something happened in the to Child A. The other children said that Child A cried when Child A?s head hit the ?oor.
