The Tennova Pain Management Center af?liated with Physicians Regional Medical Center and
Turkey Creek Medical Center (the "Center?) is committed to providing pain management
services responsibly and safely. After 30 days of your receipt of this letter, we no longer plan to
provide long-term opiate medication to our patients.

While pain medication therapy is widely used, non-opiate alternatives can be equally effective
and can be generally safer for the patients who use them. The Center in Knoxville will continue
to provide effective and compassionate treatment with non-opiate options including non-
opioid pain medications; interventional procedures such as injections and radiofrequency
ablations; referral to neurology and spine specialists; physical and aquatic therapy; weight loss
strategies; acupuncture; massage therapy; and lifestyle counseling programs.

We hope you will continue as a patient of the Center as we work together to address your
healthcare needs. If Opiate pain medications are currently being prescribed for you by our
practice, we will work with you to deveIOp a clinically-appropriate tapering regimen to
transition you to the appropriate alternatives. If, despite our best efforts, you experience
withdrawal please seek treatment at the nearest emergency department.

Again, we appreciate the opportunity to be involved in your medical care and hope you will
allow us to continue in the treatment of your pain. However, we understand you may wish to
consult with another provider. If that is the case, a comprehensive list of providers can be
found on the TN Department of Health website using the following web address:
s.health.tn. ov Facilities Listin facilities.htm. If you do decide to seek care in
another setting, in order to aid in the transition of your care, the Center will provide copies of
your medical records upon request. Until that time, please know we will continue to be
available to provide pain management services in a manner consistent with the clinical

   

approach outlined above.
