IT IS ORDERED THAT:
1. The Court finds, pursuant to Section 16-86-111, C.R.S., that the defendant is ?incompetent to proceed.?

2. The Defendant is committed to the care and custody of the Colorado Department of Human Services (CDHS)
for treatment directed toward restoring competency.

3. The Colorado Mental Health Institute at Pueblo (CMHIP) will coordinate the treatment process, as follows:

EIDefendant is in CDHS Custody: The defendant is already in CDHS custody, and shall remain in this
custody until such time as CDHS submits a report stating that, in their opinion, the defendant is restored
to competency. The Sheriff is ordered to pick up the defendant within 72 hours, or at a later time if
arranged through CMHIP, and return the defendant to the custody of this county?s jail.

EDefendant is in Jail: The defendant is committed to the custody of CDHS for restoration treatment.
CMHIP shall contact the Sheriff to arrange a date for transport of the defendant to the location of the
CDHS restoration treatment program, and transport the defendant to that location on that date.

EIDefendant is On Bond or Summons, now Ordered into Inpatient Care: The defendant now resides
at and the contact person and telephone number for scheduling admission is . The
defendant is ordered to appear at the designated location on the date and at the time provided to the
defendant by to keep CMHIP informed of any telephone or address changes by calling 719-546?
4834; and to provide his own transportation to and from the designated location.

4. CDHS is ordered to provide the Court with restoration reports every 90 days, as set forth in Section 16?85-
116, C.R.S. CDHS shall provide an original and two copies of the evaluation report to the Court at least five
days prior to the review date, and to provide copies to both parties.

5. This matter is set for review on August 11,2016.
