JKLM Energy Services – PADEP Status Report (March 9, 2016) Reese Hollow Pad 118 – 2HU Release and Response MA006627 The purpose of this status report is to provide a written update and information to the PADEP regarding activities recently completed for the subject release response and to forecast near-term activities. COMPLETED ACTIVITIES AND RECENT RESULTS: V. 1. 1 1.) Sampling of Water Sources and Other Locations: a. No new water source locations have been sampled since the last report. b. Routine Bi-weekly (twice per month) sampling continued (as access was provided by owners) as follows: i. 4 sampling points for: Charles Cole Memorial Hospital (CCMH) - 2 Supply Wells (not operating), CCMH Spring, and Hershey Pond. Since the Coudersport Borough 2 supply wells are now operating with PADEP approval, they have been dropped from the routine JKLM sampling roster. They would be sampled by JKLM if a concern arose. OP EN _S OU RC E ii. 5 formerly foam-impacted properties (Stahley-209 Fernwood Ln., Kline-228 Fernwood Ln., McCutcheon-247 N. Hollow Rd., Wagner-195 N. Hollow Rd., and Kocher-273 N. Hollow Rd.). iii. Andrews-283 N. Hollow Rd., Pulizzi-307 Burrows Rd. (only sampled when property is occupied), and Redcay-303 N. Hollow Rd. Wetzel-463 Center Park has not been sampled per the owner’s request to stop sampling and 701 N. Hollow Rd. (Mrs. Grace Redcay) has been dropped from the routine sampling per PADEP’s recent official determination of non-impact for this location. iv. 8 surface water samples from the established sampling locations. Locations are sampled as they have flow. v. JKLM has recommended to PADEP that future routine sampling frequency be reduced to quarterly. c. Analytical summary tables (via draft workbook) are attached. This table is a condensed summary primarily presenting analytes related to the release for key locations. A comprehensive spreadsheet for sample results will be issued in the next status report. d. JKLM and Penn E&R are currently preparing to resample the 4 monitoring wells on the #118 drilling pad this month. Schedule coordination is ongoing for PADEP split samples. 2.) Hydrogeologic Investigation (HI) a. Status of the 8 HI Tasks are as follows: i. Task 1 - Receptor Survey Complete. ii. Task 2 - Evalu. of Hydrogeol. Info. Complete. iii. Task 3 - Fracture Trace Analysis Complete. iv. Task 4 - Borehole Geophysics Complete. v. Task 5 - Sampling #118 2HU Complete. vi. Task 6 - MW Installation Complete. Currently Bi-Weekly. Draft Interim Hydrogeologic Report was issued to PADEP on February 26, 2016. 3.) Operating Replacement Water Systems and Water Treatment System Installations a. The 5 replacement water systems continue to operate. b. Status regarding the installation of the 6 treatment systems is as follows: i. 195 N. Hollow (Wagner), 247 N. Hollow (McCutcheon), 209 Fernwood (Stahley), 228 Fernwood (Kline), and 273 N. Hollow (Kocher) - Systems are installed, analytical samples have been collected to complete the field testing. Results for the testing will be reviewed with PADEP at scheduled March 16, 2016. After PADEP approval, owners will return to use of wells via treatment systems. ii. 125 N. Hollow (Smith) - System was delivered and is being installed by owner’s contactor, as requested by owner. Analytical testing will begin after installation is complete. JKLM is waiting to hear back from the owner. 4.) Regulatory Coordination a. Next PADEP and JKLM meeting is scheduled for March 16, 2016. PLANNED FUTURE ACTIONS: JKLM continues routine sampling. Sampling now being conducted twice per month. MWs on the pad will be resampled this month with PADEP collecting split samples. PADEP and JKLM meeting is scheduled for March 16, 2016.
