Answer 1: The Justice Center can only provide requested information on the cases the Justice Center is
solely responsible for because iocal District Attorneys are not required to report to the Justice Center
when they are prosecuting a case that would be within ouriurisdiction. in 2015, Justice Center

prosecutions increased by one, from 16 to a total oi 17 when the Justice Center agreed to handle an
additional case at the request ola locai DA.

 

Answer 3:
a Death involved cases closed in 2014 is 25; there was at least one substantiated
allegation in 10 of these cases.
a Death involved cases closed in 2015 is 53; there was at least one substantiated
allegation in 27 of these cases.

Please note that a determination that an allegation is substantiated does not mean staff conduct caused
or contributed to the individual's death. it could involve a breach ofa duty that was likely to result in

injury, but there was no conclusive rnedical evidence that it caused or contriouted to the death or statl
failure to report an incident.
