Residential Home Exterior Power Wash Cleaning Consent

Environmental Technologies, Inc. paying for the exterior power washing/cleaning of
my home at the address indicated above. My granting this consent, I am freely and
willingly agreeing to hold MAX harmless for any damage caused by the power
washing/cleaning and agreeing that payment for the power

washing/ cleaning is not an admission that its Yukon facility operations in any way
caused the need for power washing/ cleaning.

1 further agree not to use the act of payment for power washing/ cleaning of
my home as evidence that its Yukon facility operations in any way caused the need
for power washing/ cleaning at any public meeting, public hearing, comment or
complaint to South Huntingdon Township officials, PADEB or USEPA, or any media
outlet (including but not limited to newspaper, radio, televiSio'n, :01? social media]
nor in any testimony given at a deposition, magisterial district court, Common Pleas
court or any other court of law.
