Provided 6-17-16 1516 RTKL 0181 PHILADELPHIA SCHOOL DISTRICT Office of School Safety SUBJECT: I. Directive #12 PERSONAL DATA POLICY A. Use of Personal Data Summary Form 1. The Personal Data Summary Form will be used by all School Police personnel to record all personal information and any changes. Any, and all, changes should also be forwarded to Human Resources. B. Telephone Numbers 1. All personnel will furnish the Administrative Lieutenant of School Safety with their current telephone number. 2. Personnel not having a telephone in their residence must supply the Administrative Lieutenant of School Safety, with a telephone/cell phone number where you can be reached without delay. C. Military Status 1. All School Police personnel must furnish their military/reserve status when completing a Personal Data Summary Form. 2. Whenever there is a change in military/reserve status, the Divisional Commander of the employee is to be notified by submitting a change in their Personal Data Summary Form. 3. The below information shall be submitted: a. Name, rank, badge, social security number and assignment of employee. b. Branch of service attached to; c. Duty station assigned to; d. Any military occupational skill (M.O.S.) 4. If necessary, a memorandum may be used and attached to the completed Personal Data Summary Form. D. Information Changes 1. When a change of address, telephone or name occurs, a Personal Data Summary Form is submitted, the unit recall file will be updated immediately. 1 - 12 II. PROCEDURES FOR CHANGES A. The following procedures will be followed for changes: 1. Notification of change of address will be given to the Administrative Lieutenant of School Safety at least 24 hours before such change is to take effect. 2. Notification of change of phone number will be submitted to the Administrative Lieutenant of School Safety within 24 hours after the change takes place. 3. Notification of military/reserve status will be given to his/her pertinent Commander when an employee is assigned to a school/unit, within 72 hours before the change becomes effective by using the Personal Data Summary Form. 4.
