As part of its commitment to provide cleaner seas around the United Kingdom, the MCA publishes, via ACOPS, an annual survey of reported discharges attributed to vessels and offshore oil and gas installations operating in the United Kingdom’s Exclusive Economic Zone (UKEEZ). The annual survey results complement historical data and records and enable MCA's Counter Pollution and Response (CPR) team to monitor trends in the number of incidents, the amount of oil and chemicals spilt, the geographical distribution of spills, the sources of pollution and the nature of the pollution. Following analysis of 1,793 incident reports and supporting information, 748 accidental discharges from vessels and releases from offshore oil and gas installations were identified in the survey area during 2014. An 11.6% increase was evident in the total number of incidents reported from all sources over the previous year’s total. A total of 146 vessel-sourced discharges were reported during 2014 which was 2 more than the previous year’s total and significantly fewer than the mean annual total of 189 reported discharges between 2000 and 2013. A total of 601 accidental releases were reported by offshore oil and gas installations operating in the open sea during 2014 representing a 14.5% increase on the previous year’s total. Further analysis of these statistics showed that the total of 380 oil releases from offshore oil and gas installations during 2014 was the highest recorded since 2000 and 101 more than the mean annual total of 279 releases reported between 2000 and 2013. Estimated volumes of releases showed that 70% were less than 5 litres. The total of 221 non-mineral oil releases by offshore installations during 2014, primarily those involving chemical substances, was 11 more than the total recorded during the previous year. The reported incidents from all sources during 2013 comprised 68.6% mineral oils, 29.3% chemicals (including oil-based mud) and 2.1% other substances (including vegetable and animal oils). Approximately 88 % of all accidental discharges and releases were reported the open sea, 9% were in ports and harbours and 3% were across the remaining marine environmental zones. Survey respondents submitted details of a total of 70 incidents involving accidental discharges from vessels and oil rigs operating in ports and harbours during 2014 which was 33 fewer compared with the mean annual total of 103 discharges between 2000 and 2013. Page 1 of 42 Nine beach pollution incidents were reported during 2014 which was the highest total recorded since 2003 and 2 more than the mean annual total of 7 incidents reported between 2000 and 2013. The most extensive incident occurred between January and March 2014 after generally small quantities of solidified rancid palm oil residues were washed ashore in south-west and southern England including the Isles of Scilly, Cornwall, Devon, Dorset, Isle of Wight, Hampshire, West Sussex and East Sussex. The residues ranged in size and form from small nuggets to boulder-sized chunks up to 1metre in diameter. Clearance operations were organised by some local authorities and private beach owners and warning notices were erected advising dog owners of the potential hazards to their pets. Estimated volumes of accidental discharges and releases were recorded in 758 incident reports (including ongoing releases), or 94% of the total. The modal class for oil discharges and releases was less than 455 litres but larger estimated volumes of between 456 litres and approximately 30 tonnes were recorded in 9 of the 11 survey enumeration areas. In total, 6% of all oil discharges and releases (including ongoing releases) were greater than 455 litres, when this information was recorded, compared with 8.7% during the previous year. The largest oil incident, with an estimated maximum volume of 30 tonnes, occurred after the French trawler Le Sillon (160.9 gt, built 1987) ran aground and broke up on rocks in rough seas at High Butter Cove near Porthcothan on 2 February. The modal class for chemical incidents was also less than 455 litres but 36 releases, or 16% of the total (including 9 ongoing releases), were of 1 tonne or more. The largest chemical incident involving a release of 38, 896.5 kg Oceanic HW540 v2 was caused by a subsea control module leak on the Strathspey Manifold on 4 June. The leak was stopped following replacement of well components and system modifications. At least 25 confirmed discharges were attributed to vessel casualties including 13 pleasure craft and workboats, 10 fishing vessels, a supply vessel and a tug. Six vesselcasualty spills occurred in nearshore waters, 16 in ports or harbours and the remainder in the open sea. The standby safety vessel OMS Pollux (436 gt, built 1976) collided with a pile supporting a turbine at the Walney Wind Farm located off Barrow-in-Furness on 14 August. The vessel was damaged over the port side fuel tank causing a diesel oil leak, initially at a rate of 10 litres per minute. The vessel remained offshore away from environmentally sensitive areas until temporary repairs had been completed and the leak had been stopped. No reports were received of any prosecutions brought by the competent authorities for marine pollution offences during 2014. _________________________________________________ Page 2 of 42 SECTION 1: INTRODUCTION AND SURVEY DESIGN 1.1 INTRODUCTION 1.1.1 Since 1965 the Advisory Committee on Protection of the Sea (ACOPS) has compiled statistics and other information on different types of marine pollution in the waters around the British Isles. The competent authorities and other stakeholder groups have used this information for a wide range of purposes including policy and planning decisions, evaluations of particular programmes, assessments of regulatory compliance levels, development of risk assessment models and to support educational campaigns. Examples of organisations that have consulted the survey data include the Department for Transport, European Commission, International Maritime Organisation, Local Authority Associations, Marine Management Organisation, the Scottish Government and the Welsh Assembly Government. 1.1.2 The Maritime and Coastguard Agency (MCA) exercises Central Government's statutory responsibilities for taking action when oil or other hazardous and noxious substances emanating from any at-sea activity threatens the United Kingdom or its surrounding waters. The survey results complement historical data and records and enable MCA's Counter Pollution and Response (CPR) Team to monitor trends in the number of incidents, the amount of oil and chemicals spilled, the geographical distribution of spills, the source of pollution and the nature of the pollution. 1.2 SURVEY DESIGN 1.2.1 In accordance with the MCA's survey specification a two-page questionnaire was designed to capture information relating to discharges of oil and other hazardous and noxious substances from vessels and unpermitted oil and chemical releases from offshore oil and gas installations. The survey area included the UKEEZ and adjacent areas of jurisdiction including ports, harbours and tidal stretches of rivers and estuaries. The boundaries of the UKEEZ are shown in Figure 2. The survey questionnaire and accompanying guidelines are provided in Appendix 2. 1.2.2 The survey questionnaire was distributed to the appropriate representatives of coastal local authorities, port and harbour authorities, national environment agencies and animal welfare groups. In addition, questionnaire responses were supplemented with Petroleum Operation Notices (PON1s) provided by the Department of Energy and Climate Change’s (DECC) Offshore Environment Unit and Pollution Reports (POLREPS) provided by the MCA. PON1s are submitted to DECC by offshore oil and gas installations operators for all unpermitted oil or chemical releases to sea regardless of the volume lost. The resulting information was entered into a Page 3 of 42 database identifying separate pollution incidents by date, location and other survey attributes. 1.2.3 Following analysis of data the survey results were presented in a range of different formats. In Section 2 descriptive statistics are provided for reported discharges and releases including some numerical trends with 2000 as the base year and also including categories and types of polluting substances, geographical locations and distributions of incidents, amount and extent of reported pollution, sources, details of prosecutions brought by the competent authorities and reporting effort. 1.2.4 In Section 3 summary reports are provided for all larger discharges and releases of 2 tonnes or more. Further information is provided in Section 4 for each of 11 geographical areas around the United Kingdom including the causes of particular incidents and counter-pollution responses where appropriate. 1.2.5 Elements of the survey quality assurance programme are outlined in Section 5 including the number of incident reports compiled from information submitted by two or more reporting organizations. Moreover, the current database is provided in Appendix 1 as part of the quality assurance programme in order to ensure the reliability and accuracy of the survey results. This is further enhanced by posting the survey report on the MCA’s website as a publicly available document for scrutiny by all interested stakeholder groups. _______________________________________ Page 4 of 42 SECTION 2: DESCRIPTIVE STATISTICS AND TREND ASSESSMENTS 2.1 NUMERICAL TRENDS IN REPORTED DISCHARGES 2.1.1 Following analysis of 1,793 incident reports and supporting information, 748 separate discharges from vessels and releases from offshore oil and gas installations were identified within the survey area during 2014. Further details are provided in Appendix 1. 2.2 CATEGORIES OF REPORTED DISCHARGES 2.2.1 In Table 1 statistics are provided showing the different categories and numbers of discharges and releases reported during 2013 and 2014 for each of 11 enumeration areas. Geographical boundaries of each enumeration area and the complete survey area are shown in Figure 2. It should be noted that the boundary of the complete survey area was enlarged during 2014 to encompass the newly designated United Kingdom’s Exclusive Economic Zone (UKEEZ) (Refer Statutory Instruments 2013 No.3161 Marine Management The Exclusive Economic Zone Order 2013). The statistics for discharges and releases in the United Kingdom Continental Shelf (UKCS) Oil and Gas enumeration area have been presented in two source categories: offshore oil and gas installations, and vessels. Page 8 of 42 2.2.2 The discharges and releases from all sources reported during 2013 comprised 68.6% mineral oils, 29.3% chemicals (including oil-based mud) and 2.1% other substances (including vegetable and animal oils). During 2014 there was a single report of a garbage incident following a discharge from a Russian warship operating in the North Sea (Refer 4.1.6). 2.3 LOCATIONS OF REPORTED DISCHARGES 2.3.1 The geographical location of each reported discharge or release has been identified in Appendix 1 together with other information for each incident. To facilitate comparisons with previous years’ data, reported locations of all vessel-source oil discharges in the open sea, nearshore waters and estuaries during 2013 are plotted in Figure 3. Data for ports have been grouped separately for England, Wales, Scotland and Northern Ireland. 2.3.2 The overall geographical pattern for vessel-source mineral oil discharges in the survey area during 2014 showed fewer reports of incidents across 5 enumeration areas, with the highest density of reported incidents in the open sea occurring in the vicinity of North Sea oil and gas installations. 2.3.3 The geographical locations of all discharges and releases are further analysed by their distribution across five marine environmental zones in each enumeration area: open sea; tidal river or estuary; bay or nearshore waters; beach or shore; and port or harbour. The results are presented in Table 2, which shows that 87.6% of all discharges and releases were reported in the open sea, 9.4% in ports or harbours and 3% across the remaining zones. 2.3.4 Reporting organisations submitted details of 70 incidents involving discharges of polluting substances from vessels and oil rigs located in ports or harbours during 2014, which was 33 fewer than the mean annual total of 103 discharges between 2000 and 2013. The largest port incident occurred at Hartlepool on 29 March following a 1.5 tonne hydraulic oil spill from the diving support vessel Seven Falcon (11,071 gt, built 2011) (Refer 4.1.4). 2.3.5 Nine beach pollution incidents were reported during 2014 which was the highest total recorded since 2003 and 2 more than the mean annual total of 7 incidents reported between 2000 and 2013. Further details are provided in Section 4. 2.4 AMOUNT AND EXTENT OF REPORTED POLLUTION 2.4.1 Information describing observed dimensions and volumes of pollution for each enumeration area is presented in Table 3. It should be noted that the extent of pollution for incidents occurring within the UKCS enumeration area 11 was determined by the observed approximate release area of water surface pollution in 2 categories: up to 1.6 km2; or above 1.6 km2. Estimates for volumes of discharges and releases were not confirmed in written reports for 45 incidents, or 6% of the total reported (including ongoing releases). Page 12 of 42 2.4.2 Excluding releases from offshore oil and gas installations and vessels operating in the UKCS area 11, approximately 1% of oil slicks or sheens were greater than 1.6 km in length when this information was reported. Broadly comparable statistics from the 2000, 2005 and 2010 surveys were 12%, 7% and 10% respectively. Within the UKCS enumeration area 11 a release area of water surface pollution by oil greater than 1.6 km 2 was reported on a single occasion. 2.4.3 The modal class for all oil incidents was less than 455 litres but larger estimated maximum volumes of between 456 litres and 30 tonnes were recorded in 9 of the 11 survey enumeration areas. The leak was stopped following system modifications and replacement of well components (Refer 3.1 UKCS/1418). 2.4.6 In Section 3 details are provided for all reports involving releases or discharges of 2 tonnes or more during 2014 including 3 ongoing incidents that commenced during previous years. 2.5 CATEGORIES AND TYPES OF POLLUTION 2.5.1 Statistics showing the different categories and types of polluting substances are presented in Table 4. Mineral oils were again the most frequently recorded category of polluting substances and were identified in 68.6% of incident reports. Crude oils accounted for 8% of the identified types of mineral oil discharges, this figure being 6% less than the previous year’s. The largest crude oil incident, involving a release of 0.381 tonnes, was reported by the Beryl B platform on 20 May. The incident was caused by human error after the tilt-phase separator valves were left open when a unit was being recommissioned (Refer 11- UKCS/1385). 2.5.2 Bunker, diesel, fuel and gas oil incidents were attributed to vessels and oil and gas installations on 153 occasions, 26 more than the previous year’s total, across all enumeration areas with the exception of eastern England. The spills were reported most frequently in the UKCS, Irish Sea and south-west England enumeration areas. Page 14 of 42 2.5.3 Lubrication and hydraulic mineral oil incidents were recorded in 8 of the 11 survey enumeration areas with 168 releases attributed to offshore oil and gas installations. Of these, 41 hydraulic releases in quantities of up to 0.009 tonnes were identified by DECC as Known Third Party reports and attributed to remote-operated vehicles (ROVS). 2.5.4 Respondents described the remaining types of mineral oil discharges or releases as bilge oils, oily water, petrol, weathered or semi-weathered residues, sludge, well bore hydrocarbons and various waste oils. Local authorities reported extensive beach pollution by weathered or semiweathered oil residues on 3 occasions during 2014. Bournemouth Borough Council provided details of an incident in which blobs of oil were washed ashore along a 4 km stretch of coastline during the third week of January (Refer 4.4.8). Another local authority report provided details of an incident in which weathered oil residues were washed ashore along a 5 km stretch of the Thanet coastline between Margate and Ramsgate during the second week of February. Clean-up operations were undertaken by local authority personnel and a number of oiled seabirds were reported (Refer 4.3.5). A SEPA report referred to weathered oil balls observed along the shoreline between Brora and Golspie during the last week of February (Refer 4.10.8). 2.5.5 A total of 235 reports referred to incidents involving substances other than mineral oils during 2014 compared with 222 reports during 2013. Offshore operators reported 217 unpermitted chemical releases and 4 vegetable oil releases during 2014 compared with 209 chemical releases and a single vegetable oil release during the previous year. The identified substances were associated with exploration and production operations and included ammonium bisulphite, barite, bentonite, calcium chloride, chlorine, caustic soda, glycol solutions, hydraulic fluids, methanol, potassium chloride brine and sodium solutions. The estimated volumes of unpermitted chemical releases ranged from 0.001 kg to 38,896.5 kg and 36 releases (including ongoing releases) were 1 tonne or more. Further details are provided in Section 3 and Appendix 1. 2.5.6 Various chemicals need to be used as part of offshore exploration and production operations. The vast majority of chemicals spilled are regulated under the provisions of the Offshore Chemicals Regulations 2002 (OCR) for normal operational use and discharge to sea. Under the provisions of OCR each operational chemical is assessed to determine its potential environmental impact. Of the total chemicals spilled in 2014 approximately 73% were considered under the Commission for the Protection of the Marine Environment of the North East Atlantic (OSPAR) list of substances used and discharged offshore as Posing Little or No Risk to the Environment (PLONOR). None of the chemicals spilled was included as a substance listed in the OSPAR list of chemicals for priority action. None of the spill incidents gave risk to a significant environmental impact. 2.5.7 A total of 14 non-mineral oil incidents were attributed to identified or suspected vessel discharges in ports or in the open sea. Palm oil residues were washed ashore on 6 occasions in eastern England, southern England, Page 15 of 42 south-west England, Irish Sea and western Scotland during 2013. The most extensive incident occurred between January and March 2014 after generally small quantities of solidified rancid palm oil residues were washed ashore in south-west and southern England including the Isles of Scilly, Cornwall, Devon, Dorset, Isle of Wight, Hampshire, West Sussex and East Sussex. The residues ranged in size and form from small nuggets to boulder-sized chunks up to 1-metre in diameter. Clearance operations were organised by some local authorities and private beach owners and warning notices were erected advising dog owners of the potential hazards to their pets (Refer 4.5.7). 2.5.8 Respondents also submitted details of 4 incidents involving discharges of coal dust, garbage, sewage and a damaged packaged chemical cargo. Floating coal dust residues originating from a hold washings discharge were reported in Tees Bay on 29 September. A POLREP referred to a discharge of partially incinerated garbage from the Russian guided missile destroyer Admiral Levchenko operating in the North Sea on 7 January. Human error during a maintenance operation was identified as the cause of a 200-litre sewage discharge from the nuclear fuel carrier MV Ocean Pintail (5,271 gt, built 1987) at the Port of Barrow. A Tier Two response was initiated by ABP Southampton following a chemical spill from the containership CMA CGM Columba (131,332 gt, built 2010) on 3 November. Approximately 20 tonnes of Aqualure 900 leaked from a bladder container onto the vessel and shore during a cargo transfer operation. An unknown quantity of the mildly toxic water soluble liquid subsequently overflowed from the vessel into the sea (Refer 4.4.6). 2.6 REPORTED SOURCES OF POLLUTION 2.6.1 The figures in Table 4 show for each enumeration area the numbers of incidents in which reporting organisations either identified a specific source of pollution or suspected a vessel or offshore pollution source. The statistics for the identified sources of pollution are further analysed in Table 5 to show the different types of vessels. Additional details including the names of vessels or installations are provided in Appendix 1. 2.6.2 This method of analysis showed that releases from oil and gas installations operating on the UKCS accounted for 87.1% of all identified reports of pollution compared with 87% during the previous year. Corresponding statistics for fishing vessels, pleasure craft, general cargo vessels and the remaining types of vessels were 2.9%, 2.6%, 1.4% and 5.8% respectively. 2.6.3 Reporting organisations identified discharges from tankers on 8 occasions during 2014 compared with 5 occasions during the previous year. Port and harbour authorities reported tanker incidents at Buckie, Birkenhead and 2 separate incidents at the Tetney Monobuoy in the Humber Estuary. The largest port incident occurred at Birkenhead on 26 April following a 200-litre bunkering spill from the inland waterways tanker Mersey Endurance (872 gt, built 2012) (Refer 4.7.2). A clean-up operation was completed over a twoday interval and no further actions were required. Four tanker discharges were reported in the open sea during 2014. Severe weather damage to a hatch Page 16 of 42 cover caused a 1.8-tonne vacuum oil spill from the oil products tanker Maersk Rosyth (22,184 gt, built 2003) in the Dover Strait on 21 October (Refer 4.3.4). 2.6.4 Respondents also submitted details of 9 discharges from warships operating in the open sea and in ports and harbours including Barrow, Portsmouth Naval Base and Plymouth. The largest incident occurred on 12 January following a 1-tonne oily water discharge from the Invincible-class light aircraft carrier Illustrious (built 1978). The pollution was broken up and dispersed by harbour boats (Refer 4.4.4). 2.6.5 At least 25 confirmed discharges were attributed to vessel casualties including 13 pleasure craft and workboats, 10 fishing vessels, a supply vessel and a tug. Six vessel-casualty spills occurred in nearshore waters, 16 in ports and the remainder in the open sea. Moreover, vessel casualties accounted for 50% of all confirmed discharges attributed to fishing vessels and likewise 72% of all discharges attributed to pleasure craft and workboats. The corresponding statistics from the 2013 survey were 48% and 36% respectively. Further details identifying the likely causes of vessel casualties, where known, are provided in Section 4 and Appendix 1. 2.6.6 In the open sea the standby safety vessel OMS Pollux (436 gt, built 1976) collided with a pile supporting a turbine at the Walney Wind Farm located off Barrow-in-Furness on 14 August. The vessel was damaged over the port side fuel tank causing a diesel oil leak, initially at a rate of 10 litres per minute. The vessel remained offshore away from environmentally sensitive areas until temporary repairs had been completed and the leak had been stopped. A multi-agency Environment Group was set up to respond to the incident and surveys undertaken by local authorities indicated that no oil arrived ashore (Refer 4.7.3). The tractor tug Cormilan (207 gt, built 1981) ran aground at the entrance to Fowey Harbour on 29 April. The vessel was holed on the starboard side and began taking on water. A small quantity of light oil was discharged into the harbour during a pumping out operation and booms were placed around the vessel to contain and absorb the pollution (Refer 4.5.5). 2.6.7 Seven vessel-casualty spills were reported by the competent authorities in Scotland during 2014. Six incidents involved fishing vessels with a single reference to an incident involving a pleasure craft. The trawler Shalimar (168 gt, built 1985) collided with a pier and sank in Scrabster Harbour on 30 April. A boom was deployed and approximately 40-litres of oil were later recovered from the water surface (Refer 4.10.6). The 9-metre cabin cruiser Lady Russell ran aground in Proudfoot Bay near Wick on 29 October causing a small diesel spill. All potential pollutants were later removed from the vessel (Refer 4.10.7). 2.7 PROSECUTIONS 2.7.1 No reports were received of any prosecutions brought by the competent authorities for marine pollution offences during 2014 (Refer Table 6). Page 21 of 42 SECTION 3: SUMMARY OF LARGER INCIDENTS (TWO TONNES OR MORE) DURING 2014 (Refer to Section 4 and Appendix 2 for further details of each incident) The figures given below are indicative of the reported volumes of releases confirmed in written reports. However, incidents in which significant amounts of beach material were mixed with pollutants, or where the volumes released were not indicated or measured, are not included. The incidents are ordered according to the volume of the discharge or release. In all cases where the source of the release was from subsea control equipment the fluids lost were associated with oil and gas exploration and production operational processes. The operator continued the release to maintain the integrity and functionality of the wells/equipment and dive support vessels could be resourced to complete the necessary repairs. Each release required the operator to assess the potential environmental impacts associated with the release. The operator in each case was required to provide regular updates on their management plan as part of investigations being conducted by DECC. Where necessary, meetings with the operator took place to discuss aspects associated with the incident, environmental impact and the operator’s short- and long-term plans to stop the release and, where relevant, effect a final repair. 3.1 UKCS 4 June: CNR International Strathspey Manifold (11-UKCS/1418) CNR International reported an incident resulting in the release of 38,896.5 kg of Oceanic HW540 v2 due to a subsea control module leak on Well M2Z on the Strathspey Manifold. Following the successful replacement of the Strathspey well M2Z SCM, system modifications and a period of monitoring, CNRI are confident that, with the current subsea configuration, HW540 losses to sea have been resolved. 3.2 South-West England 2 February: MFV Le Sillon (5-SW/049) The French trawler Le Sillon (160.9 gt, built 1987) lost power and steering during heavy weather off Trevose Head and later ran aground at High Butter Cove near Porthcothan. Approximately 28 tonnes of diesel oil, 2 tonnes of lubrication oil and an 8-tonne cargo of fish were lost to sea after the vessel broke up on rocks in rough seas. 3.3 UKCS 13 June: Ensco plc Ensco 101 (11-UKCS/1436) Ensco plc reported an incident resulting in the combined release of 17,394.28 kg of DUO-VIS, caustic soda, M-I GEL, EMI-2224, M-I WATE (all grades), POLYPAC (all grades), SAFE-CIDE, soda ash, sodium bicarbonate, citric acid and potassium chloride brine due to an equipment and procedural failure with the control of fluid circulation on the Ensco 101 mobile drilling unit. The drilling unit was on contract to BP Exploration Operating Company Ltd and was operating in the Marnock (ETAP) field at the time of the incident. Following the incident DECC Inspectors visited the installation on two occasions to identify the cause of the release and to verify the corrective Page 22 of 42 actions taken. A number of procedural and equipment changes were made by BP and Ensco. The investigation concluded that the corrective actions taken should minimise the likelihood a future similar occurrence. 3.4 UKCS 4 October: EnQuest plc Don SW&W Don Northern Producer (11UKCS/1625) EnQuest plc reported an incident resulting in the release of 10,984.81 kg of Castrol Transaqua HT2 as a result of a subsea hydraulic release from the Don SW&W Don Northern Producer installation. DECC carried out an investigation into this incident and as a result identified several corrective actions to be undertaken. EnQuest are working closely with DECC to ensure these actions are carried out accordingly to allow this incident to be closed. DECC continue to monitor the incident. 3.5 UKCS 3 May: CNR International Ninian Central (11-UKCS/1350) CNR International reported an incident resulting in the release of 10,814.4 kg of Oceanic HW540 v2 due to a leak of subsea hydraulic control fluid from the system controlling pipeline SSIVs from the Ninian Central installation. CNRI currently have a subsea intervention vessel on location and are looking to resolve the leak during this workscope. 3.6 UKCS 29 October: Apache North Sea Forties FA (11-UKCS/1561) Apache North Sea reported an incident resulting in the release of 7.7 tonnes of diesel due to a split bunkering hose from the Forties FA installation. An investigation was undertaken. Apache have implemented satisfactory measures which should prevent a recurrence of this type of incident. 3.7 UKCS 12 September: Chevron North Sea Limited Alba Northern (11UKCS/1592) Chevron North Sea Limited reported an incident resulting in the release of 7.5 tonnes of diesel due to an overflow of the fire pump diesel day tank on the Alba Northern installation. Following an investigation by DECC, an Enforcement Notice was served requiring the operator to carry out a number of actions. This work was noted by DECC as complete in August 2015. 3.8 UKCS 21 September: Shell UK Limited Gannet D Subsea Completion (11-UKCS/1607) Shell UK Limited reported an incident resulting in the release of 7,077 kg of methanol due to the failure of a synthetic hose fitting from the Gannet D Subsea Completion. The source has been isolated pending installation of a permanent solution. 3.9 UKCS 13 January: North Atlantic Drilling UK Limited West Phoenix (11-UKCS/1209) North Atlantic Drilling UK Limited reported an incident resulting in the combined release of 6,954 kg of barite, Baracarb (all grades), potassium chloride brine, Baro-Lube-NS, GEM GP, Drispac® Superlo ™ Polymer, Flowzan® Biopolymer, DEXTRID E, SOLTEX, STEELSEAL and caustic soda due to a slip joint leak from the West Phoenix installation. The drilling unit was on contract to Total E&P UK Limited and was operating in the Page 23 of 42 Laggan field at the time of the incident. DECC carried out an investigation into this incident. Through their own investigations Total identified several corrective actions to be undertaken. The Department followed these actions up offshore and subsequently closed the inspection. 3.10 UKCS 13 May: BP Exploration Operating Company Ltd Foinaven FPSO Petrojarl (11-UKCS/1372) BP Exploration Operating Company Ltd reported an incident resulting in the release of 6,731 kg of Oceanic HW540 v2 due to a failure of the subsea control module from the Foinaven FPSO Petrojarl installation. The system was isolated to prevent further release, pending repair. 3.11 UKCS 16 December: BP Exploration Operating Company Ltd Foinaven FPSO Petrojarl (11-UKCS/1626) BP Exploration Operating Company Ltd reported an incident resulting in the release of 5,600 kg of NC-5009 due to the failure of a bunkering hose from the Foinaven FPSO Petrojarl. The hose was replaced for new and a review of operating procedures undertaken. 3.12 UKCS 31 December: Marathon Oil LLC Brae Alpha (11-UKCS/1646) Marathon Oil LLC reported an incident resulting in the release of 4,588 kg of Oceanic HW443ND due to valve failure on the subsea chemical systems from the Brae Alpha installation. The Department carried out an investigation into this incident and identified corrective actions which Marathon Oil undertook. This incident, and investigation, was closed in November 2015. 3.13 UKCS 23 August: Taqa Bratani Tern Alpha (11-UKCS/1566) Taqa Bratani reported an incident resulting in the combined release of 4,535 kg of TEG (all grades) and CRW85733 due to the failure of a temporary pipe repair clamp on the heating system from the Tern Alpha installation. The temporary repair clamps were evaluated and replaced with more permanent fittings. 3.14 UKCS 11 November: Premier Oil UK Limited Balmoral FPV (11UKCS/1587) Premier Oil UK Limited reported an incident resulting in the release of 4,358 kg of Pelagic 100 production control fluid due to integrity issues within the Balmoral subsea hydraulic control systems. A number of actions were taken to identify the source of the leak and remediate including topside pressure tests and subsea ROV surveys. Any leaks found were repaired or the associated system was isolated. 3.15 UKCS 11 January: North Atlantic Drilling UK Limited West Phoenix (11-UKCS/1208) North Atlantic Drilling UK Limited reported an incident resulting in the combined release of 3,597.16 kg of Flowzan® Biopolymer, caustic soda, DEXTRID E, Drispac® Plus Superlo™ Polymer, GEM GP, Baracarb (all grades), barite; STEELSEAL, SOLTEX, Baro-Lube-NS and potassium chloride brine due to mud flow through the wireline stuffing box from the Page 24 of 42 West Phoenix. The drilling unit was on contract to Total E&P UK Limited and was operating in the Laggan field at the time of the incident. DECC carried out an investigation into this incident and as a result of their own investigation Total identified several measures to be taken to prevent recurrence allowing the incident to be closed. 3.16 UKCS 11 August: Marathon Oil LLC Birch Manifold (11-UKCS/1550) Marathon Oil LLC reported an incident resulting in the release of 3,341 kg of Oceanic HW443N due to a poor connection on the hydraulic hose fitting on the Centrica Resources Ltd Birch manifold. This PON1 was subject to a formal DECC investigation which found that the release was attributed to loose MPA Hydraulic ‘Hose – SCM’ fitting on Z3 well. This was replaced during a DSV campaign in May 2015. 3.17 UKCS 19 August: Britannia Operator Limited Britannia (11UKCS/1558) Britannia Operator Limited reported an incident resulting in the combined total release of 3,298.8 kg of TEG (all grades) and EC1389B from the cooling medium system on the Britannia installation as a result of a bursting disc rupture upon restarting the system. DECC served an enforcement notice on Britannia Operator Limited which required the company to undertake a number of actions by a specified date. Britannia Operator Limited provided evidence of completion of these actions within the required timeframe, enabling closure of this matter. 3.18 UKCS 30 July: unidentified vessel (12-UKCS/1820) The Nelson installation reported an oil sheen with an estimated maximum volume of 3.05 tonnes which was attributed to an unidentified passing vessel. 3.19 UKCS 9 August: Marathon Oil LLC Birch Manifold (11-UKCS/1544) Marathon Oil LLC reported an incident resulting in the release of 2,860 kg of Oceanic HW443N due to a failure of the hydraulic hose on the Centrica Resources Ltd Birch Manifold. This release was part of a DECC investigation and the failed hose was replaced during a DSV campaign in May 2015 which allowed reinstatement of the MP B line. 3.20 UKCS 18 January: Marathon Oil LLC Birch Manifold (11-UKCS/1213) Marathon Oil LLC reported an incident resulting in the release of 2,599 kg of Oceanic HW443ND from the subsea hydraulic system on the Centrica Resources Ltd Birch Manifold. DSV operations have confirmed that the leak has been stopped with full pressure re-applied to system with no further leaks. 3.21 UKCS 8 May: unidentified vessel (12-UKCS/1806) The Ninian installation reported an oil sheen with an estimated maximum volume of 2.58 tonnes which was attributed to an unidentified passing vessel. 3.22 UKCS 31 December: ENI UK Limited Hewett 48/29-C (11-UKCS/1645) Page 25 of 42 ENI UK Limited reported an incident resulting in the release of 2,540 kg of Aqualink 300F Ver2 as a result of a subsea release of hydraulic fluid from the Hewett 49/29-C installation. An investigation was undertaken. ENI have shut in the well that was the source of the release and will not produce from this well until repairs are undertaken. 3.23 UKCS 29 March: ConocoPhillips Annabel Subsea Manifold (11UKCS/1300) ConocoPhillips reported an incident resulting in the combined release of 2,492 kg of Aqualink 300E Ver2 and Aqualink 300F Ver2 due to a loose connection on the subsea infrastructure from the Centrica North Sea Gas Annabel Subsea Manifold. A ROV investigation visually identified a loose connection as the source of the leak. A DSV subsequently returned to the field and affected a repair using divers. 3.24 UKCS 22 February: Marathon Oil LLC Brae Alpha (11-UKCS/1245) Marathon Oil LLC reported an incident resulting in the release of 2,461 kg of Oceanic HW443ND due to an incorrect valve line up during testing from the Brae Alpha installation. DECC carried out an investigation into the incident and corrective actions were undertaken by Marathon Oil to prevent recurrence. An offshore inspection was undertaken in August 2015 to confirm the valve line up. 3.25 UKCS 10 December: Nexen Petroleum Scott JD (11-UKCS/1623) Nexen Petroleum reported an incident resulting in the combined release of 2,078.914 kg of EC6202A, ammonium bisulphite 65%, FOAMTREAT 943, OS15 and sodium hypochlorite (up to 15% w/w) due to split water injection pipeline on the Scott JD installation which released treated seawater to the sea. The pipeline was shut in during December 2014 and is no longer in use. 3.26 UKCS 13 September: Transocean John Shaw (11-UKCS/1595) Transocean reported an incident resulting in the combined release of 2,030.2 kg of M-I WATE (all grades), calcium chloride (all grades), Truvis, Versaclean CBE, Ecotrol RD and lime. The incident occurred after a blockage behind a valve caused an overflow of the flowline which ultimately allowed drilling mud to release to sea from the Transocean John Shaw drilling rig. The butterfly valve that caused the blockage has been replaced with a more suitable valve and inspection hatches were installed in the flow line to allow for better detection of blockages. 3.27 UKCS 17 February: unidentified vessel (12-UKCS/1800) The Magnus installation reported an oil sheen with a maximum estimated volume of 2 tonnes which was attributed to an unidentified passing vessel. UKCS INCIDENTS OCCURRING IN A PREVIOUS CALENDAR YEAR BUT WITH QUANTITIES RELEASED DURING 2014 The following reported UKCS incidents occurred during a previous calendar year, but still continued over a period of time during 2014. Page 26 of 42 3.28 UKCS 1 January: EnQuest Heather Limited Don SW and West Don Northern Producer (ongoing incident from 8 December 2011) EnQuest Heather Limited reported an ongoing loss of 48,176.01 kg between 1 January and 31 December 2014, of Gyptron SA3460 scale inhibitor from a subsea jumper leak on the Don SW and W Don Platform. DECC continue to investigate this incident and work with EnQuest to ensure a resolution is reached. The work scope is still planned for 2015 to resolve the incident. Impact assessments continue to be carried out and any effects have been determined to be minimal. 3.29 UKCS 1 January: EnQuest Heather Limited Don SW and West Don Northern Producer (ongoing incident from 14 March 2013 UKCS/1285) EnQuest Heather Limited reported an ongoing loss of 11,588.51 kg between 1 January and 31 December 2014, of Castrol Transaqua from a leaking fitting on the West Don PO3 wing valve. DECC are investigating this incident and are working in conjunction with EnQuest to ensure a satisfactory conclusion. A subsea diving campaign has been scheduled for 2015. 3.30 UKCS 1 January: EnQuest Heather Limited Don SW and West Don Northern Producer (ongoing incident from 20 June 2012 UKCS/1510) EnQuest Heather Limited reported an ongoing loss of 8,191 kg between 1 January and 1 September 2014, of Castrol Transaqua HT2 as a result of a leak on the Don South West Subsea Riser Base Valve on the gas lift line. Six discharges occurred in local ports and harbours and 5 discharges were in nearshore waters and the open sea. A source of pollution was identified on 9 occasions, the sources including 2 oil products tankers, 2 fishing vessels, a diving support vessel, an offshore construction vessel, a Russian destroyer, a chemical tanker and a pleasure craft. Releases from offshore oil and gas installations operating off north-east England were included in the statistics for the UKCS (Refer to 4.11). 4.1.2 Three incidents were reported in the River Humber area during 2014. A POLREP referred to a 2-litre crude oil spill from the oil products tanker Elisabeth Knutsen (71,850 gt, built 1997) at the Tetney Monobuoy on 24 March. The vessel was discharging cargo and ballasting when there was a valve failure, resulting in an overboard discharge. The cargo discharging operation was temporarily suspended until the source of pollution had been identified. The resulting sheen was broken up and dispersed by a tug in attendance at the stern of the vessel. A leaking thruster gland was identified as the cause and source of a lubrication oil discharge from the oil products tanker Sallie Knutson (87,827 gt, built 1999) at the Tetney Monobuoy on 1 August. The necessary repairs were completed and the resulting light surface sheen was dispersed by wave action. A Hull City Council report referred to a 25-litre diesel oil discharge from a pleasure craft on 21 August. A clean-up operation was completed by marina staff. 4.1.3 An act of vandalism was identified as the cause of a diesel spill from the 10metre wooden-hulled fishing vessel Northern Pride (built 1977) at Warkworth Harbour. To help prevent a recurrence of the incident the harbour authority upgraded the existing CCTV to cover a larger area. 4.1.4 A POLREP and a PD Teesport report referred to a 1.5 tonne hydraulic oil spill from the diving support vessel Seven Falcon (11,071 gt, built 2011) at Hartlepool on 29 March. The incident occurred after a crane hose split during a maintenance operation and a clean-up was completed. An unidentified vessel was believed to have been the source of a diesel sheen observed on the River Tyne. 4.1.5 An oil seal failure on a pile driving hammer caused a 10-litre hydraulic oil spill from the offshore construction jack-up vessel MPI Resolution (14,310 gt, built 2003) operating at the Humber Gateway Wind Farm. Floating coal dust residues originating from a hold washings discharge were reported in Tees Bay on 29 September. 4.1.6 Three incidents were reported in the open sea during 2014. The Russian guided missile destroyer Admiral Levchenko (DD-605) was identified as the source of a discharge of partially incinerated garbage off Flamborough Head on 7 January. On 19 October a surface sheen was observed trailing in the Page 29 of 42 wake of the Danish chemical tanker Maria Thereza (2,659 gt, built 2002) off the Humber estuary. Subsequent communications with the vessel determined a permissible discharge of tank washings. An unknown quantity of diesel oil was lost to sea after the 15.8 metre long twin-rig trawler Ocean Way sank off the Northumberland coast on 2 November. 4.2 Eastern England (Lincolnshire, Norfolk & Suffolk) 4.2.1 Four incidents were recorded during 2014, one less than the total recorded during the previous year. Accidental releases from offshore oil and gas installations operating off eastern England were included in the statistics for the UKCS (Refer to 4.11). 4.2.2 Two incidents were reported in the open sea during 2014. The Leman installation reported an oil sheen attributed to an unidentified vessel on 5 August. The offshore supply vessel Gargano (2,244 gt, built 2002) operating in the southern North Sea reported an ongoing vegetable oil leak at the rate of 10 litres per day. The vessel later entered a dry dock where repairs to a defective thruster seal were completed. 4.2.3 A POLREP described an incident at the port of King’s Lynn on 23 October following an accidental discharge of engine room washings from the MV Portunas moored at the Boal Quay. The oil was quickly dispersed on a strong ebb tide. 4.2.4 Beach clearance operations were completed by local authority personnel at Great Yarmouth after small quantities of palm oil residues were washed ashore during the last week of November. The source of pollution was believed to have been a discharge from an unidentified passing vessel. 4.3 Essex and Kent 4.3.1 Nine vessel-source pollution incidents were identified by reporting organisations during 2014 compared with 13 incidents during 2013. Five discharges occurred in local ports and harbours, 3 discharges in the open sea and there was one reference to a beach pollution incident. The source of pollution was identified on 8 occasions and there were 5 references to fuel oils as polluting substances. 4.3.2 Port and harbour authorities reported vessel discharges at Dover, Harwich, London and Medway. A leaking stern tube seal was identified as the cause of hydraulic oil discharge from the RO-RO ferry Spirit of France (47,592 gt, built 2011) inside Dover Harbour on 20 November. Propeller wash from a harbour patrol launch dispersed the pollution. The vessel was subsequently dry-docked and the seal replaced. The converted multi-purpose catamaran work boat Offshore Wenduine reported a 75-litre marine gas oil discharge at the Port of Harwich on 3 May. The incident occurred during a bunkering operation and a clean-up was completed with the assistance of the oil spill recovery vessel Haven Hornbill. Page 30 of 42 4.3.3 Absorbent booms, pads and bilge socks were deployed in a counter pollution operation at London’s St Katharine Docks following a diesel oil discharge from the sightseeing touring barge Josephine on 1 July. A burst pipe caused a 10-litre hydraulic oil spill from the vehicle carrier African Highway (23,850 gt, built 1982) alongside the Sheerness Car Terminal. Propeller wash and an absorbent boom were deployed in a clean-up operation. The PC Bowmanteig sank in the Port of Medway on 12 April and an absorbent boom was deployed until the vessel was recovered and removed. 4.3.4 The oil/chemical tanker Flagship Iris (29,669 gt, built 2012) was observed discharging unidentified tank washings in the open sea off the North Foreland on 4 March. The MV Celtic Explorer reported a 20-litre gas oil discharge in the Outer Thames Estuary on 4 August. Severe weather damage to a hatch cover caused a 1.8-tonne vacuum gas oil spill from the oil products tanker Maersk Rosyth (22,184 gt, built 2003) in the Dover Strait on 21 October. 4.3.5 Weathered oil residues were washed ashore along a 5 km stretch of the Thanet coastline between Margate and Ramsgate during the second week of February. Clean-up operations were undertaken by local authority personnel and a number of oiled seabirds were reported. 4.3.6 In addition 10 seabirds were found covered in fuel oil in Ramsgate Harbour during the first week of April. 4.4 Southern England (East and West Sussex, Hampshire, Dorset and the Isle of Wight) 4.4.1 Reporting organisations submitted details of 19 pollution incidents attributed to vessels during 2014 compared with 17 incidents during 2013. Sixteen recorded incidents were in ports and harbours compared with 10 during the previous year. A source of pollution was identified on 18 occasions including 5 pleasure craft, 5 warships, 4 fishing vessels, 2 barges, a general cargo vessel and a containership. In addition to the incidents above reporting organisations also provided local details relating to a larger-scale incident affecting the coastline of southern and south-western England (Refer 4.5.7) 4.4.2 Oil and chemical discharges resulting mainly in surface sheens were reported in local ports and harbours at Fishbourne, Hamble, Littlehampton, Lymington, Poole, Portsmouth, Shoreham, Southampton and Weymouth. A diesel sheen was reported after the 9-metre fishing vessel Katy Lou sank whilst berthed alongside Fishbourne Quay on the Isle of Wight. The vessel was refloated the following day. On 31 March the 11-metre sailing yacht Fortyniner caught fire and sank at the Mercury Yacht Harbour on the River Hamble. Harbour staff deployed a boom in a clean-up operation. A split pipe caused a 4-litre hydraulic oil spill from the modular jack-up barge Haven Seajack I (78.8 gt) at Littlehampton on 17 October. Absorbent mats and rolls were deployed in a counter pollution operation. Page 31 of 42 4.4.3 Lymington Harbour Commissioners provided details of 2 pollution incidents during 2014. A contaminated bilge water discharge from an unidentified pleasure craft was believed to have been the source of a diesel sheen reported at the Borthon Marina. A hydraulic line failure caused a 5-litre discharge from a pleasure craft at the same location on 18 August. A 40-metre pleasure craft was identified as the source of a 10-litre diesel oil discharge at Poole Harbour’s Sunseekers Quay on 5 June. The incident occurred during an internal fuel oil transfer operation and a clean-up operation was later completed. An automatic bilge pump-out was identified as the cause of an oily water discharge from a fishing vessel in Poole Harbour on 2 August. 4.4.4 The Queen’s Harbour Master (QHM) Portsmouth provided details of 5 oil pollution incidents during 2014. The Invincible-class light aircraft carrier Illustrious (built 1978) was identified as the source of a 1-tonne oily water discharge following a tank overflow on 12 January. The oil was broken up and dispersed by harbour boats. Containment and absorbent booms were deployed in a counter pollution operation following a 200-litre waste oil discharge from the Type 45 Destroyer HMS Duncan (built 2010) on 30 October. A misaligned fuel pipe was identified as the cause of a 20-litre fuel oil spill from the Hunt-class mine countermeasures vessel HMS Middleton (built 1983) on 22 January. A faulty tank gauge was believed to have been the cause of a 10-litre diesel oil discharge from the Duke-class Frigate HMS St Albans (built 2000) whilst alongside the Fountain Lake Jetty. The pollution was broken up and dispersed by propeller wash from a tug and a pilot boat. A split pipe caused a 20-litre waste oil spill from a barge on 24 January. 4.4.5 A Shoreham Port Authority report referred to a 5-litre hydraulic oil spill from the MV Muhlenau (2,461 gt, built 2004) on 10 February. The incident occurred after a hydraulic hose to the hatch system burst allowing oil to spill onto the deck and overboard through the scuppers. The Shoreham Port Oil Spill Team carried out a clean up operation following a 15-litre bunkering spill from the stern trawler Marina Emiel and all costs, including waste disposal, were charged to the owners of the vessel. 4.4.6 A Tier Two response was initiated by ABP Southampton following a chemical spill from the containership CMA CGM Columba (131,332 gt, built 2010) on 3 November. Approximately 20 tonnes of Aqualure 900 leaked from a bladder container onto the vessel and shore during a cargo transfer operation. An unknown quantity of the mildly toxic water soluble liquid subsequently overflowed from the vessel into the sea and slowly dispersed until no longer visible. The wooden-hulled pleasure craft Mary Gloag sank at her pontoon mooring at Weymouth Harbour’s Westwey Road Marina on 25 January. Over several days approximately 90 litres of diesel escaped from the submerged vessel and this was left to disperse naturally. 4.4.7 In nearshore waters a diesel sheen and debris were observed after the 10metre fishing boat Dulce T suffered an ingress of water and sank in the Needles Channel near the Shingles Bank. The pollution was later broken up and dispersed by wave action. In the open sea off the Isle of Wight the Type Page 32 of 42 23 Duke-class frigate HMS Kent (built 1998) reported a small fuel oil spill which was quickly broken up and dispersed by wave action. 4.4.8 Bournemouth Borough Council provided details of an incident in which blobs of oil were washed ashore along a 4 km stretch of coastline during the third week of January. 4.4.9 In addition to the pollution incidents outlined above several reporting organisations provided details of a lost cargo recovery and beach clearance operation. The containership Svenborg Maersk (92,198 gt, built 1998) enroute from Rotterdam to Sri Lanka lost 517 containers overboard whilst crossing the northern stretch of the Bay of Biscay in stormy conditions on 14 February. Searches undertaken by the MCA’s surveillance aircraft located 4 containers and one of these was later washed ashore at Axmouth. During late February and March the contents of other containers including cigarettes, pet food and packets of tea were washed up along the Dorset coastline. Clearance and recovery operations were organised by several agencies including HM Revenue & Customs, UK Border Force, loss adjustors for Maersk, Environment Agency (EA), Natural England, local authorities, Dorset Coast Forum and the Dorset Waste Partnership. More than 40 tonnes of materials recovered at sea and on the shoreline were later incinerated for electricity production. 4.4.10 In an unrelated incident several containers filled with methanol were recovered on Brighton beach during the first week of February. 4.5 South West England (Devon, Cornwall, Isles of Scilly, Channel Islands and the Western Approaches) 4.5.1 Sixteen vessel-source pollution incidents were identified by reporting organisations during 2014 compared with 18 incidents during 2013. Seven discharges occurred in ports and harbours, 2 in the open sea, 5 in nearshore waters and estuaries and there were also 2 references to beach pollution incidents. A source of pollution was identified on 10 occasions, the sources including 4 pleasure craft, 3 fishing vessels, a patrol vessel, a tug and a warship. 4.5.2 Reporting organisations provided details of 2 incidents in the open sea during 2014. An overfilled fuel tank was identified as the cause of a 5-litre diesel discharge from the River-class offshore patrol vessel HMS Mersey (built 2003) on 22 April. A sea boat was deployed to break up and disperse the pollution by propeller agitation. A passing fishing vessel reported sighting a thick diesel sheen in the English Channel off Black Head on 22 May. 4.5.3 Four oil discharges were reported in the nearshore waters of south-west England during 2014. The fishing vessel Le Sillon (160.9 gt, built 1987) lost power and steering during heavy weather off Trevose Head and later ran aground at High Butter Cove near Porthcothan. Approximately 28 tonnes of diesel oil, 2 tonnes of lubrication oil and an 8-tonne cargo of fish were lost to sea after the vessel broke up on rocks in rough seas. A total of three oil slicks Page 33 of 42 extending for up to 800 metres in length were reported in Whitsand Bay on 13 September and 18 October. Two slicks were identified as diesel oil and the third was described as thick black oil. Two leaking drums lost from an unidentified passing vessel were identified as the source of a surface slick reported off Plymouth on 26 November. 4.5.4 Seven oil spills were reported in ports and harbours at Bideford, Brixham, Falmouth, Fowey and Plymouth. The wooden-hulled fishing vessel Pacemaker (49 gt, built 1970) was identified as the source of a diesel sheen in Bideford Harbour on 29 May. POLREPS described 2 incidents in Brixham Harbour during 2014. Absorbent booms and pads were deployed in a cleanup operation following a bilge discharge from a fishing vessel alongside the fish quay. On 30 August a pleasure craft sank in Brixham’s Outer Harbour and a small diesel sheen was later dispersed by harbour staff. 4.5.5 Absorbent pads were used in a clean-up operation at Falmouth after a pleasure craft sank on her mooring in the Inner Harbour on 16 August. Three reporting organisations submitted details of an incident after the tractor tug Cormilan (207 gt, built 1981) ran aground at the entrance to Fowey Harbour on 29 April. The vessel was holed on the starboard side and began taking on water. A small quantity of light oil was discharged into the harbour during a pumping out operation and booms were placed around the vessel to contain and absorb the pollution. As a precautionary measure the booms remained in place until repairs to the vessel’s hull had been completed. 4.5.6 Two oil spills were recorded in Plymouth Harbour during 2014. A motor boat caught fire and sank on 22 January causing a small diesel sheen which later dispersed naturally. The Daring-class air-defence destroyer HMS Dauntless (built 2007) reported a 5-litre diesel oil spill in the same location on 23 May and a clean-up operation was later completed. 4.5.7 Two beach pollution incidents were reported during 2014. Between January and March generally small quantities of solidified rancid palm oil residues were washed ashore in south-west and southern England including the Isles of Scilly, Cornwall, Devon, Dorset, Isle of Wight, Hampshire, West Sussex and East Sussex. The residues ranged in size and form from small nuggets to boulder-sized chunks up to 1-metre in diameter. Clearance operations were organised by some local authorities and private beach owners and warning notices were erected advising dog owners of the potential hazards to their pets. A similar but smaller scale incident was reported in November after solidified palm oil residues were identified on some beaches in North Devon, East Devon and South Devon. 4.6 Bristol Channel and South Wales (Somerset, Bristol, Gloucestershire, Monmouthshire, Newport, Cardiff, Vale of Glamorgan, Bridgend, Neath Port Talbot, Swansea, Carmarthenshire and Pembrokeshire) 4.6.1 Three incidents were reported during 2014 compared with 4 incidents during 2013. Two discharges occurred in ports and one in nearshore waters. Page 34 of 42 4.6.2 An EA report described an incident after the PC Iselin T sank in Bristol City Docks on 6 January causing a small diesel sheen. A boom was deployed and the vessel was raised and removed the following day. 4.6.3 On 22 April a thin surface sheen with an estimated maximum volume of 10 litres was reported off Highveer Point and later dispersed naturally. 4.6.4 The charter boat Haf-Aled heeled over in Fishguard Harbour at low tide allowing diesel oil to escape through breather holes in a fuel tank. The pollution was dispersed naturally on the incoming tide. 4.7 Irish Sea (Ceredigion, Gwynedd, Isle of Anglesey, Conwy, Denbighshire, Flintshire, Merseyside, Lancashire, Cumbria, Isle of Man and all Districts of Northern Ireland) 4.7.1 Respondents identified 19 discharges from vessels during 2014 compared with 4 discharges during the previous year. Eleven incidents occurred in ports and harbours, 6 in the open sea or estuaries and there were 2 references to beach pollution incidents. A source of pollution was identified on 13 occasions and 5 spills were caused by vessel casualties. Details of accidental releases from oil and gas installations operating in Morecambe Bay are included in the statistics for the UKCS (Refer to 4.11). 4.7.2 Eleven incidents were reported in the north-west England area during 2014. A floating pontoon was identified as the source of a 50-litre diesel spill into the River Mersey on 3 February and absorbent booms were deployed in a clean-up operation. Human error during a maintenance operation was identified as the cause of a 200-litre sewage discharge from the nuclear fuel carrier MV Oceanic Pintail (5,271 gt, built 1987) at the Port of Barrow on 17 February. The inland waterways tanker Mersey Endurance (872 gt, built 2012) reported a 200-litre diesel spill after a hose split during a bunkering operation at Birkenhead. The harbour authority’s emergency pollution response team completed a clean-up operation over a two-day interval and no further actions were required. An overflow from a fuel tank was identified as the cause of an accidental 800-litre diesel discharge from the MFV New Venture (10.02 gt) after the vessel heeled over at low tide in Workington Harbour on 10 August. An EA report referred to a diesel spill from an unidentified vessel in Fleetwood Harbour. The Astute-class nuclear powered submarine HMS Artful (launched 2014) was identified as the source of a 1litre hydraulic oil spill at the Port of Barrow’s Wet Dock Quay on 30 November. 4.7.3 In the open sea the standby safety vessel OMS Pollux (436 gt, built 1976) collided with a pile supporting a turbine at the Walney Wind Farm located off Barrow-in-Furness on 14 August. The vessel was damaged over the port side fuel tank causing a diesel oil leak, initially at a rate of 10 litres per minute. The vessel remained offshore away from environmentally sensitive areas until temporary repairs had been completed and the leak had been stopped. Overflights conducted by the MCA’s counter pollution surveillance aircraft identified a light surface sheen approximately 1 km long trailing the Page 35 of 42 vessel. A multi-agency Environment Group was set up to respond to the incident and surveys undertaken by local authorities indicated that no oil arrived ashore. The Douglas installation reported 2 oil sheens during 2014 which were attributed to unidentified passing vessels. 4.7.4 Local authorities in north-west England provided details of 2 beach pollution incidents during 2014. A Copeland Borough Council report referred to a clearance operation to remove approximately 1 m 3 of solidified palm oil from a beach at Beckermet near Sellafield during the second week of July. Small quantities of solidified palm oil were also washed ashore at Morecambe on 17 December. 4.7.5 Four discharges, including 3 vessel-casualty spills, were recorded in the ports and harbours of mid-Wales and North Wales during 2014. A slight sheen was reported after the PC Daybreak sank in Holyhead Marina on 20 February. An 8-metre motor cruiser caught fire and broke up at Aberystwyth and the resulting diesel sheen was dispersed naturally. A small sheen was observed after the 6-metre cabin cruiser Mystery Dawn sank at Aberdovey. In addition, the motor cruiser Avanti sank in the River Teifi estuary on 2 March and a small quantity of diesel oil was later observed around the vessel and on the shore. The vessel was later recovered and the oil dispersed naturally. The windfarm support vessel Gardian 3 (43 gt, built 2011) reported a 15-litre hydraulic oil discharge following an oil cooler failure at the Port of Mostyn. An oil spill response plan was activated and the spill was contained. Natural Resources Wales later issued a warning letter to the owners of the vessel, Gardline Environmental Limited, for causing pollution. An invoice was also sent to the company to recharge for the costs incurred during the pollution investigation. 4.7.6 Two incidents were reported in the open sea off mid-Wales and North Wales during 2014. A diesel sheen was reported in the vicinity of the Gwynt Y Mor Windfarm on 1 May and was attributed to a discharge from an unidentified vessel. The product tanker Nordic Hanne (24,112 gt, built 2007) reported a 200-litre lubrication oil sludge discharge in the St George’s Channel on 10 November. The spill occurred whilst transferring fuel on deck to a generator and the pollution was quickly broken up and dispersed by wave action. 4.7.7 A POLREP and a Northern Ireland Environment Agency report referred to a 500-litre heavy fuel oil discharge from the RO-RO cargoship Seatruck Panorama (14,759 gt, built 2008) at Warrenpoint Harbour on 22 October. The harbour authority initiated a Tier One response in which booms and absorbents were deployed and some shoreline pollution was reported. 4.8 Western Scotland (Dumfries and Galloway, Ayrshire, Inverclyde, Argyll & Bute and the Highlands & Islands Region to Cape Wrath) 4.8.1 Reporting organisations submitted details of 9 vessel-source pollution incidents during 2014 compared with 13 during the previous year. Five oil discharges were reported in local ports or harbours and 3 oil discharges in nearshore waters and the open sea. A single beach pollution incident was Page 36 of 42 also reported. Further analysis of the information contained in annual returns showed 2 references to oil discharges caused by vessel casualties. 4.8.2 Reporting organisations submitted details of oil discharges at Acairseid, Cairnryan, Craugure, Oban and Troon. Oil spill contractors were mobilised to carry out a counter pollution and salvage operation after the MFV Quest sank alongside a pontoon in Acairseid Harbour on the Isle of Eriskay. Approximately 1 tonne of diesel oil leaked from the submerged vessel over several days and booms were deployed to contain the pollution. A salvage operation was successfully completed on 11 March. A Dumfries and Galloway Council report referred to an 800-litre heavy fuel oil spill from the RO-RO ferry Stena Superfast VII (30,285 gt, built 2001) at Cairnryan. A clean-up operation was completed by a Tier Two responder and the cause of the incident was believed to have been equipment failure during a bunkering operation. 4.8.3 A Scottish Environment Protection Agency (SEPA) report noted a 2-litre diesel oil discharge from the 16-metre workboat Usk Moor (built 1984) at Craugnure Pier on 18 October. A 20-litre bunkering spill from an unidentified vessel was reported at Oban’s North Pier on 11 April. A POLREP described a 40-litre light hydraulic oil discharge from the catamaran ferry P&O Express (5,902 gt, built 1998) in Troon Harbour. The resulting surface sheen dispersed naturally and no further actions were required. 4.8.4 The skipper of the Caledonian MacBrayne RO-RO car ferry Loch Fyne (549 gt, built 1991) reported an accidental discharge of 16 litres of hydraulic gear oil in the Sound of Mull on 8 May. The oil dispersed naturally and the vessel was later taken out of service for the necessary repairs. In the open sea a faulty propeller shaft seal was identified as the cause of a 500-litre lubrication oil spill from the Ro-Ro ferry European Highlander (21,188gt, built 2002) on 20 March. It was believed that the discharge occurred over an extended period of time in the North Channel whilst the vessel was en-route to Larne from Cairnryan. The 13.8 metre creeler Osprey (32 gt. built 1965) sank off Cape Wrath on 23 July. A thin slick of diesel oil was later observed above the wreck site. 4.8.5 A small quantity of solidified palm oil was washed ashore on a beach at Carsethorn on 7 April. In response Dumfries & Galloway Council issued a warning that the material was potentially fatal if ingested by dogs. 4.8.6 In addition the MV Isis (674 gt, built 1978) reported as having lost 30-40 one-tonne logs near the Corrran Narrows on 7 January. At least 10 logs were washed ashore and the remainder were recovered during a search operation undertaken by local vessels. 4.9 Orkney and Shetland Islands 4.9.1 Respondents identified 6 oil discharges during 2014 compared with 5 discharges during the previous year. Three incidents occurred in ports and Page 37 of 42 harbours and 3 in nearshore waters and the open sea. A source of pollution was identified on 5 occasions including 3 fishing vessels, a crew boat and a pleasure craft. Accidental releases from oil installations operating in the West Shetland Basin were included in the statistics for the UKCS (Refer to 4.11). 4.9.2 Orkney Islands Council Marine Services provided details of 3 incidents in local ports and harbours during 2014 and on each occasion a clean-up operation was completed and no further actions were required. The 18-metre fishing vessel Celtic Dawn II (105 gt, built 2005) was identified as the source of a 10-litre lubrication oil discharge at Stromness on 4 April. The 21metre crew boat Herston Lass (75 gt, built 1993) reported a 5-litre diesel oil discharge at Flotta on 17 October. Approximately 100-litres of diesel oil were accidentally discharged into Kirkwall Harbour from the pleasure craft Adlergrund. 4.9.3 Two vessel-casualty spills were reported in the nearshore waters of Shetland during 2014. The 12-metre long wooden scallop dredger Diamond 2 sank after hitting a rocky outcrop east of Snarra Ness on 25 March. A 20-metre long diesel slick emanating from the wreck was quickly dispersed by wave action. A 9-metre creeler sank off Whiteness on 20 November causing a small diesel spill which later dispersed naturally. 4.10 Eastern Scotland (the northern and eastern shores of the Highlands and Islands Region from Cape Wrath, Moray, Aberdeenshire, Angus, Fife, Lothian and Scottish Borders) 4.10.1 Eighteen incidents were recorded by reporting organisations during 2014, two fewer than the previous year’s total. Analysis of incidents by marine environmental zones showed 14 spills occurred in ports and harbours, 3 in the open sea and nearshore waters and there was one reference to a beach pollution incident. The source of the pollution was identified on 15 occasions, sources including 5 fishing vessels, 4 support/supply vessels, an oil products tanker, a pleasure craft, a tug, a port tender, a passenger boat and a semi-submersible drilling rig. At least 3 incidents were known to be caused by vessel casualties which were 2 fishing vessels and a pleasure craft. 4.10.2 Aberdeen Harbour Board provided details of 4 discharges during 2014 compared with 5 discharges during 2013. A support/supply vessel was identified as the source of pollution in each incident. Estimated volumes were provided for 3 discharges and the median volume was 15 litres compared with 190 litres during the previous year. The standby safety vessel Ocean Clever (1,409 gt, built 1975) reported two minor hydraulic oil discharges on 5 May and 4 June. On both occasions the oil dispersed naturally and no further actions were required. A computer failure in the fuel gauge system was identified as the cause of a 150-litre fuel oil discharge from the supply vessel Normand Carrier (3,051 gt, built 1996) on 17 June. During a counter-pollution operation undertaken by the contractor Briggs Marine a boom was placed around the vessel and the oil recovered from the harbour. On 17 November the standby safety vessel Vos Dee 1,104 gt, built Page 38 of 42 1974) reported a 15-litre fuel oil spill during a bunkering operation at the Regent Quay. A clean-up operation was completed and human error was identified as the cause of the incident. 4.10.3 One pollution incident was reported by the Cromarty Firth Port Authority during 2014 compared with 5 incidents during 2013. The semi-submersible drilling rig Songa Dee (15,757 gt, built 1984) was identified as the source of a 20-litre diesel spill at Invergordon on 30 October. The spill occurred whilst refuelling a portable compressor and the oil dispersed naturally. 4.10.4 Elsewhere, reporting organisations recorded 7 oil spills in ports and harbours at Buckie, Firth of Forth, North Berwick, Pittenweem, Scrabster and Seton. POLREPS identified 3 oil discharges from vessels at Buckie Harbour during 2014. Absorbent booms were deployed in a counter pollution operation following a 50-litre diesel spill from 24-metre trawler Deeside (160 gt, built 1989). In a later incident booms recovered a small quantity of oil following a planned discharge of oily bilge water from the oil products tanker John Wood (362 gt, built 1968). The discharge occurred during a stern gland repair operation which required a bilge pump-out. The trawler Caledonia II (106 gt, built 2001) reported an 80-litre oily water discharge on 15 December and a clean-up operation was completed. 4.10.5 Forth Ports and SEPA recorded an incident on 10 March after a burst oil pipe caused a one-litre hydraulic oil discharge from the port tender Forth Boxer (83 gt, built 1995). A counter-pollution operation was required after a 5-litre bunkering spill from the tug Forth Drummer (114 gt, built 1967) at Burntisland East Dock. The open wooden passenger boat Sula II was identified as the source of a diesel oil spill in North Berwick Harbour on 2 June. A leak in the fuel system of the vessel was identified as the cause of the spill and absorbent booms were deployed in a clean-up operation. A Fife Council report referred to a 1 tonne-diesel discharge from a fishing vessel in Pittenweem Harbour on 30 March. 4.10.6 The trawler Shalimar (168 gt, built 1985) collided with a pier and subsequently sank in Scrabster Harbour on 30 April. A boom was deployed in the area around the submerged fishing vessel and approximately 40-litres of black bilge oil were recovered from the water surface with absorbent matting. A gantry barge was used in the subsequent salvage operation and mechanical failure was identified as the cause of the incident. The 10-metre fishing vessel River Island (8.03 gt, built 1988) sank in Port Seton Harbour on 23 September and a clean-up operation was later completed. 4.10.7 The 9-metre cabin cruiser Lady Russell ran aground in Proudfoot Bay near Wick on 29 October causing a small diesel spill. All potential pollutants were later removed from the vessel. The Buzzard and GSF Arctic III installations each reported a surface sheen attributed to an unidentified passing vessel. 4.10.8 Weathered oil balls were reported washed ashore between Brora and Golspie during the last week of February. Page 39 of 42 4.11 United Kingdom Continental Shelf 4.11.1 The statistics for this enumeration area were again compiled from POLREP reports submitted by the MCA and PON1 reports submitted by DECC. Operators are required to notify DECC where an unauthorised release of oil or chemical to sea, regardless of volume, has occurred. Each incident is reported on a PON1 to DECC’s Offshore Environmental Inspectorate, MCA, the Joint Nature Conservation Committee (JNCC) and Marine Scotland. 4.11.2 DECC reviews every PON1 that is received in order to assess the potential for environmental impact and the need for any counter-pollution response actions. In addition, each incident reported is reviewed to ensure actions are being undertaken to prevent recurrence and to assess whether there has been any contravention of legislation. This is undertaken in accordance with DECC's investigation and enforcement policies. 4.11.3 Separate data sets have been presented for releases attributed to offshore oil and gas installations and for discharges attributed to vessels, whether identified or not, operating within the UKCS area (Refer to Figure 2). 4.11.4 Oil and gas installations All unplanned releases of oil or chemicals, regardless of volume, must be notified to the relevant authorities. Releases of operational chemicals being used subsea as part of the exploration and production process may take place over a significant period of time. In some cases it may be many months before remedial work can be undertaken due to the timescales involved in sourcing vessels and equipment to carry out the work. Environmental risk assessments have shown that these events, which are normally of relatively small volume, usually cause negligible impact on the marine environment. Details of all releases, including a cause when known, are listed in Appendix 1 (UKCS oil and gas installations) and further information is provided in Section 3 for those releases of 2 tonnes or more. 4.11.5 A total of 380 oil releases were attributed to oil and gas installations operating in the open sea during 2014 including the west Shetland Basin, Liverpool Bay and off the coast of eastern England. The 2014 annual total was the highest total recorded since 2000 and 101 more than the mean annual total of 279 releases reported between 2000 and 2013. 4.11.6 Analysis of oil types showed that 44% of reported releases were identified as lubrication and hydraulic oils, followed by 24% fuel oils and 9 % crude oils. The corresponding statistics from the 2013 survey were 40%, 22% and 16% respectively. Estimated volumes for 384 oil releases were reported (including 4 ongoing releases) and of these approximately 70% were less than 5 litres. 4.11.7 In addition, the authorities were notified of 217 releases of chemical substances during 2014 compared with 209 releases during the previous year. These chemicals are routinely used in oil and gas exploration and production operations and included ammonium bisulphite, barite, bentonite, calcium Page 40 of 42 chloride, chlorine, caustic soda, glycol solutions, hydraulic fluids, methanol, potassium chloride brine and sodium solutions. 4.11.8 The estimated volumes of unpermitted chemical releases ranged from 0.001 kg to 38,896.5 kg and 36 releases were 1 tonne or more. It should be noted, however, that many of the larger discharges associated with operational chemicals being used sub-sea occurred over an extended interval due to the requirement for operators to source vessels and equipment to locate the source of the release and undertake repairs. Further details are provided in Section 3. 4.11.9 Of the total chemicals spilled in 2014 approximately 73% were considered under the OSPAR list of substances used and discharged offshore as Posing Little or No Risk to the Environment (PLONOR). None of the chemicals spilled are included in the OSPAR list of chemicals for priority action, which are considered to pose the greatest hazard. None of the spill incidents resulted in a significant environmental impact. 4.11.10 The PON1 reports again indicated that remedial actions were taken by operators following accidental releases. The responses included identification of root causes of spills and improvements in operational control procedures, recommendations concerning preventative actions and carrying out any necessary repairs and modifications to faulty or damaged equipment. Vessels 4.11.11 A further 33 incidents were identified in POLREPS and PON1s. Most involved surface sheens or thicker oil slicks reported by the nearest offshore installation but attributed to a discharge from a ‘third party’ elsewhere that was generally assumed to have been a passing vessel. The maximum estimated volumes, where known, ranged from less than 1 litre to 3.05 tonnes. In each incident the pollution was believed to have dispersed naturally without posing a threat to wildlife or the coastline. On 30 July the Nelson installation reported an oil sheen with an estimated maximum volume of 3.05 tonnes. __________________________________________ Page 41 of 42 SECTION 5: QUALITY ASSURANCE PLAN AND ACKNOWLEDGEMENTS 5.1 Quality Assurance Plan As part of a comprehensive quality assurance plan data verification procedures were again applied at all stages during the 2014 survey. For example, other sources of information were consulted and any relevant reports were cross-referenced with the appropriate questionnaire returns. The approaches for data entry and analysis were clearly shown in the relevant tables and all entries to the enumeration area sub-sets were subject to independent checking to detect any errors. On some occasions unexpected values were confirmed with the various data-sources. Multiple reports were again received from DECC and MCA describing accidental discharges from oil and gas installations operating on the UKCS. For the remaining discharges from vessels 31% of individual incident reports were compiled from questionnaire returns submitted by two or more reporting organisations which was 1% more than the previous year. As part of the quality assurance plan the complete database is provided for scrutiny in Appendix 1 thereby enabling stakeholder groups to determine the reliability and accuracy of the survey results. Reports are requested for pollution incidents attributed to discharges, or suspected discharges, from vessels and offshore installations operating within the UK Pollution Control Zone, harbour waters and national waters. Information is not required for pollution incidents attributed to land-based sources such as industrial premises, sewers and outfalls. Annual returns can be made either by email or by post. The email and postal versions of the survey form can be downloaded from the ACOPS website at www.acops.org.uk (under ACOPS Reports on the main menu) together with previous survey reports.
