individual named committed the alleged offense(s) stated in this af?davit in support of probable cause:

The facts of the alleged offense(s) were reported to me through credible source(s).

SUMMARY STATEMENT TO SUPPORT PROBABLE CAUSE:

On or about the 26th of January, 2015, while acting as the Executive Director of the Ohio State Dental
Board, Ms. Reitz did disclose to E.H., a potential target of a Dental Board Investigation, theuncovered
identity, home address and email address of the anonymous complainant along with the status of the
investigation at a time when the complaint was still under initial investigation. Complaints ?led and
subsequent investigations conducted by the Dental Board are con?dential. This disclosure occurred in
the form of email communications directly to E.H. which were subsequently discovered by the Dental
Board, provided to the IG, and subsequently followed up upon during the IG investigation.
Additionally, I conducted an interview with the Dental Board investigator who had been instructed by
Ms. Reitz to identify the previously unidenti?ed caller and report on his ?ndings; the investigator?s
statements corroborate the fact that Ms. Reitz was responsible for the email communication.
