Attachment #1 Vendor Contract #0000003566 Purpose: This contract is for hazardous material cleaning of the homeless encampments located throughout the Seattle area. This includes the containment and disposal of needles, urine, vomit and other potentially infectious human body fluids from homeless encampments. Scope of Work Scope of Work for Non -Hazardous a n d H a z a r d o u s Material Clean-Up: Scope of Work includes arriving on time at City predetermined locations, safely and appropriately collecting all forms of rubbish, garbage and other items designated for disposal, to be loaded into a vehicle, provided by the Vendor, by hand or with the assistance of too ls as needed, provided by the Vendor and delivered to a city disposal site where the vehicle will be unloaded safely and in compliance with applicable regulations, dumping the items at the expense of the City by prior authorization. For the trucking services required under the Contract, the Vendor agrees to the extent feasible and available, to consider coordinating with another City vendor, Washington State Trucking Services. The City shall provide the Vendor with contact information. Services also include the following actions: a. Screening and selection of workers suited to meet Customer's demand for work for general labor in accordance with the scope of work specified and limited to work identified. Training of workers supplied to Customer consistent with safety protocols and best practices established by the State Department of Labor & Industries. b. Training of workers supplied to Customer consistent with Vendor’s standards for customer service. c. Coordination and scheduling of workers to meet Customer's demand for work. d. Coordination of transportation of workers to job site by Vendor. e. Performance Standard/Quality checks upon completion of work and solicitation of feedback from Customer on worker performance. f. Facilitation of communication among workers and between worker(s) and Customer about performance expectations or other work-related issues. The Vendor will provide all equipment and supplies in the performance of the required scope of services. Scope of work and pricing will be identified in each request for quote. All quality control and safety requirements will be coordinated by the contractor with the City Project Manager(s). Vendor Qualifications: The vendor must: 1. Meet OSHA and WISHA regulations to clean and remove biological waste. 2. Be blood born pathogen compliant. (Employee training) 3. Provide personal protective equipment for their employees. 4. Vendor shall be mandated by WAC 296-823 and Federal Regulation 29CFR1910.1030 Reference: WAC 296-823 and Federal Regulation 29CFR1910.1030. Response Time and Requirements: The scope of work shall be performed on an on-call, as needed basis Monday through Friday during normal business hours, typically 8 a.m. to 5 pm, unless otherwise mutually agreed by Vendor and City. The City shall provide at least 24 hours advance notice to the Vendor for scope and scheduling purposes. Removal of Regulated Waste Requirements: The vendor must: 1. Discard contaminated material and regulated waste in biohazard bags, closeable, puncture resistant, or leak resistant containers. 2. Remove the containers from the location. 3. Be in accordance with applicable regulations of the United States, states and territories, and political subdivisions of states and territories. 4. Vendor shall be mandated by WAC 296-823 and Federal Regulation 29CFR1910.1030 Pricing Description of HOURLY RATES: (excludes WSST) (Overtime Hourly Rates the Same) A. Lead Technician/Supervisor - $80.00 B. Operator/Driver - $80.00 C. Labor - $80.00 ________________________________ TOTAL - $240.00 HOURLY RATE for Non-Contract Service The hourly rate for non-contract or non-scheduled service provided during Contract Normal Hours (8am-5pm) would be the same as listed above. HOURLY RATE for non-contract Service Provided outside of Contract Normal Hours The hourly rate for non-contract or non-scheduled or non-contract service provided outside Contract Normal Hours would be the same as listed above.
