He/she had his/her attorneys present. XTO personel showed up and they had Kevin Brucha, Ben Kissel, Rick Bonazza, Steve, and Matt Burger (attorney from Buchanan Ingersoll Rooney). The complainant said on Friday the 6th, the complainant's spouse was out front weeding the flowers and could hear air venting from their water well. Said the complainant's spouse starting to feel bad and they went to hospital where they found he/she had methane in blood. The complainant's attorney said they had tested him/her by a VBG test (venus blood gas) and he/she had showed a toxic affect (gas exposure). Said they contacted the well driller and he came out and found they had methane in well. He disconnected the electric and installed a 1.5" steel vent on well. XTO and the driller talked. XTO then contacted the complainant and came out and found gas in the well and in the home and gave them carbon monoxide 3 in 1 detectors to install. They left for the weekend and stayed with somebody. XTO came out Saturday morning and installed a 4" plastic vent on the well. XTO took the vent off the well and measured 0% gas in the well with PID meter. There was no noise now from the well. DEP verified the 0% reading with Model 60 meter. XTO (Rick) connected the electric with the complainant's permission. We ran the water for 15 minutes before sampling from the basement before any treatment. SAC 946 and M/E was taken. Miner took an ISO bottle just in case. We measured the water well with meter at 5 minute intervals while it was running at got 0% each time. We also left the water run for a total of 50 minutes and got another 0% reading over the water well. We then checked the house with both meters. XTO got a very low reading of .1% in pump room. The complainant asked me (Lux) if they could move back in. I said that we really couldn't advise him/her on that but that the readings all indicted that the gas levels are low to non-existent at this time. We said that we would keep them posted on lab results and periodically stop and check their well next week. XTO re-installed the vent pipe just to make sure until we get verification from the lab. 6/13/14 The lab sent the results of .026 mg/l of methane to us. Brian M. called them to let them know and Chris M. followed with another phone call later in the day. OP EN _S OU RC E On 07/11/2014, Brian MacQuarrie responded to odor complaint that was reported on 7/10/2014. While onsite no odors were observed. A zero percent free gas reading was obtained at well head. Date Assigned: 06/10/2014 Inspector: CHRISTINE J MINER eFACTS Inspection Id: Date Investigated: 06/10/2014 Type: Site Visit Violation (Y/N): N Investigation Id: 319926 Investigation Description: Visited site on 6-10-14, with Lux and MacQuarrie. Sampled for inorganic and organics. Lux, MacQUarrie and Miner performed multiple site vistis after that time frame. Results showed no effect from menthane above the action level. Minor effects above the drinking water standards for some metals. XTO informed of requirement to restore or replace supply. 3218 letter went out 12-12-14. 3218 determined there was an effect to the water supply. XTO cooperating since the beginning of the investigation. Presumptive liability, not a gas migration. cjm SSRS_CTS00121 Ver 1.3 Page 298 of 362
