Law Supplemental Narrative: Supplemental Narratives Seq Name Date Narrative 1 Howard C 10:22:45 07/09/15 Review of Documents on Supplemental, Incident Number: 14S024442 I (Detective Caitlin How ard) received the documentation from the Pueblo Regional Center in reference to 's(DOB ) death. I reviewed the nursing notes, voluntary statements and call logs from 198 Galatea Dr Pueblo West, CO, 81007. This is a summary of what information I obtained from the documents for more information please refer to original that were scanned into files. November 13, 2014, stated to staff members she was not feeling well. November 14, 2014, refused to go to work due to being ill. November 15, 2014, 2nd shift staff members came into the home and found lying in vomit and feces. 1st shift staff stated refused to let them clean her up. The 1st shift staff stated she only had vomit not feces. Those staff members were removed and placed in other homes with clients with no medical needs. November 15, 2014 at approximately 5:30 P.M., ambulance to Parkview Hospital. November 16, 2014, was transported via was pronounced deceased at Parkview Hospital. Pueblo Regional Center started their internal investigation on November 17, 2014. Staff member, HCT 1, reported the following: On November 13, 2014, had not been feeling well. November 14, 2014 refused to go to w ork. On November 15, 2014, 2nd shift staff members came into the home and found lying in vomit and feces. The first shift staff said refused to let them clean her up. The first shift staff stated she had only vomit not feces on her. The first shift staff members were removed from the home. The staff will be working at homes without complex medical needs. The staff will be retrained a standard level of care for individuals that are ill. The treatment provided, to fluids and take medication. was encouragement to take a bath, drink This is a summary of the nursing notes: November 13, 2014 at approximately 5:30 P.M., had a upset stomach, did not eat dinner, no emesis, normal output from ostomy bag. November 14, 214 at approximately 11:00 AM, had upset stomach and stated he did not want to go to day program. Temperature 99 degrees, she was encouraged to rest. November 14, 2014 at approximately 7:00 PM, still not feeling well. low grade temperature 99 degrees. Encouraged to rest and fluids. Assisted November 15, 2014 at approximately 6:30 PM, At 3:30 PM staff reported emesis and unable to keep pills down. Temperature 98.6, gave Maalox, while cleaning up she fell on left side apparent injury abrasions/bruising when addressed at 3:30 PM. At 6:00 PM staff reports multiple emesis, unsteady gait. Staff reports 's fingers are very cold but oxygen level reading at 70 percent.Instructed to call ambulance was transported to Parkview Hospital. November 16, 2014 at approximately 11:45 P.M. staff were notified as well as Director A witness statement was completed by Galatea. This is a summary of his statement. passed away at Parkview home . a staff member 198 stated after he clocked into his shift and started his daily routine he refused to get up all day. It had also been reported that had been sick and was laying in her own urine, feces and vomit.The other staff, , immediately went back and gave a bed bath, changed bedding, as well as her colostomy bag. stated when he went to give her 3:00 PM medications, he saw down in the bed covered in vomit, urine and feces. agreed to take her medication but as soon as she did a very large amount of emesis came up. 's emesis was yellow/green with no food trace. After this , continued to clean and change while he called the RN . notified that had vomited a lot and was unsure if her medication was swallowed. He told RN that, due to being covered in body fluids, he was unable to get vital signs. ordered to give Maalox for an upset stomach which was given and seemed to be tolerated. was briefed asked , the RN, to come and look at because she didn't look good. She was green, very weak and her vomit didn't smell normal. About thirty minutes ,after the Maalox was given, informed him that she was trying to change 's bedding and she fell out of the chair. was informed and came by to look at Orders were given to keep her hydrated and try broth/soup wrote, told him that did not listen or even assess for dinner. but just looked at her. At 5:00 PM, attempted to give her medication and a multivitamin. took her medication and immediately vomited a large amount of bile. This time, and attempted a full set of vitals. RN was notified immediately after we got a blood pressure of 70's/50's. and were unable to get a pulse ox reading. They attempted to get readings on her fingers, toes, even ear lobes. was on the phone with RN and she was insistent that they were doing it wrong or the machine was broken. A pulse oxygen reading of 70 percent came up. told RN that needed to go to the emergency room. RN told "The flu is not emergency status." wrote in his statement he kept trying to tell RN that was not ok and needed help.
