June 26, 2014 The incident at VT Halter resulted in two employees being hospitalized at the University of South Alabama hospital in Mobile. The employees are in critical conditions. Three other employees were injured when they were running away from the incident. The three employees were treated and released from a local hOSpital in Pascagoula. The company was using two Manitowoc cranes to move a bow section of a vessel. The bow section weighed 252 tons. The job was going to require the bow section to be stood up with the pointed end in a downward position. Each of the Manitowoc 16000 crawler cranes were rated for 440 tons. The two cranes were going to move the bow to the south. One crane was to the north of the load the other crane was to the south of the load. The cranes were new. The cranes were only 2 years old. The rear section of north crane started rising up. The boom of the north crane fell on a tool room in a north production building. The boom broke at the cab. is in critical condition. An employee in the tool room is in critical condition. The south crane was using lug to secure the crane to the deck. The lug broke. The crane boom and load block whipped to the south. The crane was not damaged. No employee was injured by this crane. Manitowoc plan to have representatives on site tomorrow. Each of the cranes had data loggers. The data loggers will have to be returned to Wisconsin to download the data. The data loggers should provide information on the cranes status and what may have caused the failure. The company had a pre-lift meeting prior to the lift procedure. The company plans required each crane to lift 126 tons. The Jackson area of?ce is requesting assisting form the regional of?ce or nation of?ce to assist in this investigation. June 27, 2014 The CSHO has copies of crane inspections records. sin an induced coma in a Mobile hospital. The doctors have removed his right eye. He also had surgery to remove pressure on his brain. 'as the employee injured in the tool room. He was having surgery one leg. Jerfonned the calculations and drawing for moving the load. The CSHO interviewed one of the riggers moving the load. A surveillance camera captured a video of the accident. The company will provide a copy of the video to OSHA. The ?le was too large to email. The video will be place on ajump drive and be given to the CSHO. The video show the two cranes moving the load together. The south crane moved south. The north crane was being repositioned or relined to travel south when the angle of the load line increase leading to the crane incident. The video also indicated there may have been communication issues between the riggers and the crane operators. Haig Engineering is on site coordinating the incident investigation for V. T. Halter. Haig Engineering requested permission to move the cab of the north crane. l-laig was concerned the crane in its current state will result in hydraulic and diesel hosed leaking ?uid. l-Iaig Engineering was bringing in a contractor from Texas to upright the crane cab. Haig Engineering estimated it will take one to two days to upright the crane cab. JAO OSHA denied the request. JAO informed Haig Engineering to keep the crane cab in its current position until national and regional OSHA representatives arrived Monday or Tuesday. There was a conference call this morning with national office, regional of?ce, of?ce of public affairs, regional solicitor office, Jackson area office and the compliance of?cer. Everyone was updated on the status of the inspection. national of?ce and regional of?ce will travel to the incident site next Monday to provide engineering support. Angela Donaldson, solicitor will travel to the site Monday to assist with interviews of management employees. June 30, 2014 Manitowoc representatives arrived on site Friday aftemoon. Manitowoc representatives will delay removing the data loggers until Tuesday awaiting the arrival of OSHA representatives. No temporary workers were involved in the crane incident. The company has temporary workers working at other locations in the facility. Evidence and Site Control Agreement was faxed and emailed to corporate safety director on Friday.
