At what time did Nissan call for an ambulance? 4. Was CPR performedwhat time and by whom? 5. At what time was the clinic called? 6. At what time did clinic personnel arrive to give medical assistance to Mr. Whiting? 7. Has Nissan made any changes to its protocols for handling such situations and similar situations since the death of Mr. Whiting? 8. Was a de?brillator used on Mr. Whiting? If so, at what time was it used and who operated it? How many operational de?brillators are there in the plant? 9. What was Mr. Whitings time of death? Was he on Nissan property at that time? 10. Has Nissan had any contact with the Madison County Coroner related to Mr. Whitings death? If so, what information has been requested or shared? 11 Does Nissan keep an emergency contact on file in HR for each employee? If so, under what circumstances does Nissan contact an employee's family? 12 Do Nissan or its suppliers have any agreement or understanding with area ambulance services regarding emergency services" 13. Why were employees required to work the remainder of their shift after this traumatic event? 14 Was any incident or other report completed related to Mr. Whitings?
