Sex/Race: Defendant. mama/m Your complainant, having been first duly sworn, on oath, states that: Count 1: INTENTIONALLY ABUSE PATIENTS - LIKELY TO CAUSE BODILY HARM The above-named defendant on or about Thursday, October 20, 2016, in the City of Marshfield, Wood County, Wisconsin, while employed as a nurse at Norwood Health Center, did intentionally pull the hair of_ a patient at that facility, an act that is likely to cause harm, contrary to sec. Wis. Stats, a Class I Felony, and upon conviction may be fined not more than Ten Thousand Dollars or imprisoned not more than three (3) years and six (6) months, or both. Your complainant is a deputy with the Wood County Sheriff's Department and his information is based upon the records and files of the Police Department. Your complainant has relied upon those records and files in the past and has found them to be both truthful and reliable. According to those records and files: 1. Marshfield Police Officer Landon Kramer states that on October 31, 2016, at 12:53 he was dispatched to the Norvvood Health Care Center located at 1600 N. Chestnut Avenue in the City of Marshfield, Wood County, Wisconsin to investigate a possible battery complaint. When Officer Kramer arrived at the Norwood Health Care Center he spoke with the complaining party, Maria Luepke, an employee. Luepke told Officer Kramer that another nurse at Norwood, Arlene Dampier, came to her earlier in the day to report that Rosemary C. Carlson, the defendant, was abusing a patient at Norwood. The defendant, at this time was a nurse assigned to the traumatic brain injury department of Norwood and was responsible for the care of patients in that department. 2. Dampier told Offioer Kramer that on October 20, 2016 at approximately 8:00 she and the defendant were taking care of patient Dampier saw the defendant pull - hair at least two times while the defendant was supposed to be caring for his needs. Dampier told Officer Kramer that the defendant pulled - hair in response to him not being cooperative with allowing the defendant to care for him. Dampier heard the defendant say to - "if you want to act like a two year old, I'll treat you like one." The defendant then pulled Service/Defendant/Agenq OF WISCONSIN VS - Ros ry Carlson - hair again. As Dampier and the defendant were leaving - room, the defendant told Dampier, "That's what you do. You pull hair so that there are no bruises." The defendant also told Dampler not to tell anyone about her pulling - hair. 3. On October 20, 2016, --was a patient at Norwood, an Inpatient healthcare facility, and as a sufferer of a traumatic brain injury, is a vulnerable adult residing in the brain injury department of the hospital. 4. Dampier told Officer Kramer that on October 21, 2016, she was again working with the defendant at Norwood. Dampier heard sounds coming from room. -was a patient in the traumatic brain injury department of NonNood at this time. Dampier walked into his room, and observed the defendant attempt to force feed -his medication by prying them into his mouth. - spit the pills onto the floor, and the defendant picked them up, and once again attempted to stuff the pills Into his mouth. While this was happening, Dampier tried to get the defendant to stop by telling her that .akes his pills with pudding, but the defendant refused to sto forcing the meditation into - mouth. As Dampler and the defendant were leaving room, the defendant told Dampier and CNA Carlie Schreiner, who was also present in the room, "Neither of you are to breathe a word about this." 5. Luepke told Offlcer Kramer that under no clrcumstances are employees to forcibly administer medications to patients. 6. Luepke told Officer Kramer that the defendant admitted to Luepke that the defendant had pulled - hair, and the defendant did not deny that she had forcibly attempted to administer medication to - Complainant Subscribed and sworn to before me on February 20, 2017, and approved for filing.
