You are directed to investigate and document the facts and circumstances concerning a fatal incident involving a Light Medium Tactical Vehicle (LMTV) belonging to 3rd Battalion, 16th Field Artillery Regiment, 2nd Brigade Combat Team, ist Cavalry Division on 2 June 2018 in the vicinity of East Range Road and Cold Springs Road (PV 309575) in the Fort Hood training area. In general, your investigative efforts must address the 5W3 (who, what, when, where, and why). You will identify any systemic deficiencies or particular noncompliance with applicable laws, regulations, programs, policies, processes, or procedures that may have affected the incident (fatal LMTV incident). You will also make recommendations as to any appropriate corrective action that should be undertaken and document any innovative programs or ?best" practices you identify during your investigation. b.
