DEPARTMENT OF THE NAW COMMANDER UNITED STATES PACIFIC FLEET 250 MAKALAPA DRIVE PEARL HARBOR, HAWAII 96860-3131 IN REPLY REFER TO: 5830 Ser 12 Jun 17 From: Commander, US. Paci?c Fleet To: Vice Chief of Naval Operations Subj: COMPREHENSIVE REVIEW OF THE AND FA-18 PHYSIOLOGICAL EPISODES Ref: VCNO memo of 21 Apr 17 Encl: (1) Comprehensive Review 1. My comprehensive review into the facts, circumstances, and processes surrounding the recent physiological episodes (PE) involving and FA-18 aircrew, as tasked by reference is included as enclosure (1). As of this writing, efforts are ongoing across the Naval Aviation Enterprise to address the PE issue. Therefore, it is likely some of the report?s recommendations are already being implemented to some degree, if not completely. 2. This is a complex issue, one without a single cause, and therefore, without a single solution. The only common thread running through all of these cases is that aircrew were physically affected. 3. Given those challenges, I believe implementation of the report?s recommendations will signi?cantly assist in identifying the causes of the physiological episodes, and bene?t our Naval Aviation Enterprise, and more importantly, our aircrew.
