Good Afternoon Alison,

Please find attached Forget-Me-Not Care and Counsellings 2015 funding application.

As previouly discussed it has proven difficult to calculate costings for the National Investagtion as we simply dont
know them, however we spoke to again this morning and she can say that nation wide we are dealing
with 150 families, therefore we have costed on these numbers, although we can't be sure of the opt-in numbers.

I have also outlined briefly the other sources of funding applied for and status.

FUNDING REQUEST

Forget-Me?Not Care and Counselling is now a Fully Registered Charity
as of 26 January 2015.

We are looking forward to supporting work of the National
Investigation come April, and we have around 12 parents moving
forwards into the next financialyear.

We have been applying to other sources for funding and to date, and
we are awaiting replies, however most of the funding is for general
bereavement and not ?Ashes? related.

Having spoken with of the NI team we are working with
numbers in excess of 150 families coming forward throughout the
country, however opt in rates are unknown, so we have estimated
expectation of an upsurge in demand from last year, so we have
projected our costs therewith.
