Goolsby, M.D. have performed an autopsy on the body of Donald Basford on the 27thday of August, 2014, commencing at 1:10 PM in the Office of the Chief Medical Examiner, Eastern Division, Tulsa, Oklahoma. Assisting in the examination is Garrett Rosser, Pathology Assistant. CIRCUMSTANCES OF DEATH: This 36 year old white male was reportedly found unresponsive seated inside a motor vehicle in a parking lot by a passerby. Emergency services were called and he was pronounced deceased at the scene. Further investigation at the scene revealed that the decedent was reportedly a transient. No information regarding past medical history is known at the time of autopsy. An autopsy was ordered. AUTHORIZATION: The postmortem examination is performed under the authorization of the Office of the Chief Medical Examiner, Eastern Division, Tulsa, Oklahoma. IDENTIFICATION: The decedent is reportedly initially unidentified at the scene and designated “U/I Male Tulsa Edison”. The decedent is subsequently positively identified by circumstantial evidence (postmortem/antemortem tattoo comparison). The decedent is received in a black plastic body bag sealed with a blue plastic tag labeled “6516771 affixed to the outside of the body bag. Two standard OCME tags are also affixed to the outside of the body bag and labeled “U/I Male Tulsa Edison, Case No. 1403829”. Photographs of the body are obtained for the purpose of identification and documentation. CLOTHING AND PERSONAL EFFECTS: The decedent is received wearing the following items: 1. 2. 3. The clothing and personal effects are returned in the body bag with the decedent to the custody of the funeral home representative transporting the body following the autopsy. IDENTIFYING MARKS, SCARS AND TATTOOS: There are multiple tattoos found on the body. No distinct marks or scars are identified. EVIDENCE OF MEDICAL INTERVENTION: No evidence of medical intervention is identified. RADIOGRAPHY: Postmortem radiographs are obtained. No radiopaque foreign bodies consistent with projectiles or retained embedded sharp implements are identified. EXTERNAL AND INTERNAL EVIDENCE OF INJURIES: No external or internal evidence of injuries are identified. EXTERNAL EXAMINATION The body is that of an unembalmed, well developed, well-nourished white male measuring 69 ½ inches in length and weighing 154 pounds, whose appearance is consistent with the reported age of 36 years. The head hair is red, straight, and measures up to 1 ¼ inch in length. The head is normocephalic. The head and upper torso are plethoric with fixed vascular congestion. The face is symmetric. The eyes are dark and decomposed. The ears, nose and mouth are unremarkable. Dark brown purge fluid is present in the nose and mouth. Natural teeth are in poor repair. The anterior torso is symmetric with a soft, mildly protuberant abdomen. The posterior torso is unremarkable. The upper and lower extremities are bilaterally symmetric. External genitalia are those of a normally developed mature male with bilaterally descended testes. Rigor mortis is passed. Livor mortis is purple-red, posterior in distribution, and fixed. The body is cool to touch. There are advanced changes of postmortem putrefactive decomposition with mummification and evidence and artifacts of insect (maggots) on the body. INTERNAL EXAMINATION The body is opened with the usual “Y”-shaped thoracoabdominal and intermastoid incisions. HEAD: The scalp is unremarkable without apparent abrasions, contusions or lacerations. The skull is intact without fracture. The meningeal coverings of the brain are intact without epidural, subdural or subarachnoid hemorrhages. CASE NO. 1403829 PAGE 4 The 1,010 gram brain is symmetric with an unremarkable gyral pattern. There are no visible injuries on the surface or cut section of the brain. The distribution of cranial nerves at the base of the brain appears normal. The cerebral vessels are unremarkable and normally distributed. The brain is in an advanced state of decomposition and degenerates with slight digital pressure. No obvious lesions or areas of hemorrhage are seen. NECK: There are no hemorrhages into the musculature or soft tissues of the neck. The hyoid, larynx and trachea are intact without obstructions. The tongue is unremarkable without injury. The cervical vertebrae are palpably intact. BODY CAVITIES: All organs are in their normal anatomic locations. The diaphragm is intact bilaterally. There are appropriate quantities of subcutaneous and intra-cavity adipose tissue, measuring up to 1.5 cm in thickness on the anterior aspect of the abdomen inferior to the umbilicus. PARIETAL PLEURA: The parietal pleural surfaces are intact, smooth and glistening, without associated adhesions. The pleural cavities contain no pleural effusions. PERICARDIUM: The pericardium is a smooth, glistening, intact membrane. The pericardial cavity contains a minimal quantity of brown decomposition fluid. PERITONEUM: The peritoneum is lined by a smooth, glistening, intact membrane. The peritoneal cavity contains minimal brown decomposition fluid. CARDIOVASCULAR SYSTEM: The aorta is patent throughout, has an even diameter, and shows no significant atherosclerosis. Its major branches are normally distributed and the ostia are patent. The pulmonary arteries show no thromboemboli or significant atherosclerosis. The vena cava and major tributaries are widely patent without thromboemboli or significant atherosclerosis. The endothelial surfaces are diffusely stained with blood pigments. The 205 gram heart has a soft, flabby consistency (comment: consistent with postmortem decomposition) and a smooth, glistening, intact epicardial surface. The right dominant coronary arteries are normally distributed and free of significant atherosclerosis. The coronary ostia are normally located and widely patent. The chambers and atrial appendages are unremarkable. The endocardium is uniformly thin, smooth, shiny and translucent. No interventricular septal defects or congenital abnormalities are seen. The myocardium is intact and uniformly red-brown without focal lesions. The left and right ventricles are 0.9 and 0.2 cm in thickness at the lateral walls, respectively, and symmetric. The chordae tendineae are intact, thin and not fused. The papillary muscles are intact and show no fibrosis. The valves show no perforations or vegetations. RESPIRATORY SYSTEM: The right and left lungs weigh 275 and 210 grams, respectively. The visceral pleural surfaces are smooth, glistening and intact with scattered moderate anthracosis and no bleb formation. The overall configuration is normal. The trachea is widely patent. The major bronchi and bronchioles bilaterally are normally formed and contain no significant occlusive material. The pulmonary arterial tree is free of emboli or thrombi. The parenchyma is dark purple-brown, and exudes moderate quantities of dark brown decomposition fluid from its cut surfaces. No focal lesions, consolidations, granulomatous or neoplastic diseases are identified. There is vascular congestion in the dependent segments. CASE NO. 1403829 PAGE 5 DIGESTIVE SYSTEM AND LIVER: The esophagus is unremarkable. The stomach is of normal configuration, empty, and otherwise unremarkable. The duodenum, small intestines, appendix and large intestines are unremarkable. The 960 gram liver has a normal configuration with a smooth, intact capsule. The parenchyma is soft, redbrown, without focal lesions. The intrahepatic ducts are not dilated. There is no nodularity or increased fibrosis. The extrahepatic biliary tree is intact and patent, free of stones and neoplasm, and are not dilated. The gallbladder lies in its usual position, has a shiny surface and its wall is evenly thin throughout. It contains a trace of bile fluid and no stones. The mucosa is without focal lesion or mass. The pancreas lies in its normal position, shows a normal configuration and is unremarkable. RETICULOENDOTHELIAL SYSTEM: The 45 gram spleen has an intact, slightly wrinkled gray capsule. The parenchyma is diffluent. There is a normal distribution of unremarkable lymph nodes. The thymus gland is involuted. GENITOURINARY SYSTEM: The right and left kidneys weigh 60 and 70 grams, respectively. Both kidneys show normal configuration without abnormality. The capsules are intact and strip with ease to reveal smooth subcapsular surfaces. The cortices are of normal thickness, with sharp corticomedullary junctions. The calices, pelves, and ureters are patent and unremarkable. The urinary bladder is normal, has a smooth mucosa, and is empty. The testes, prostate gland and seminal vesicles are unremarkable. ENDOCRINE SYSTEM: The pituitary, thyroid, and adrenal glands are unremarkable. The parathyroid glands are inconspicuous. MUSCULOSKELETAL SYSTEM: HISTOLOGY: The musculoskeletal system is intact and otherwise unremarkable. No tissue is submitted. TOXICOLOGY: The following specimens are submitted for possible toxicologic analysis: liver tissue and brain tissue. A separate report will be issued. Autopsy revealed an apparent well-developed, well-nourished adult white male with advanced changes of postmortem putrefactive decomposition with mummification and evidence and artifacts of insect (maggots) activity. No definite perimortem skin, soft tissue or skeletal injuries are identified; however, changes of advanced postmortem decomposition along with postmortem insect activity may have obscured perimortem injury, if present. Postmortem toxicology testing is noncontributory. Given the findings at autopsy, in context with information obtained from the scene investigation, the cause and manner of death cannot be determined at this time and is therefore best classified as undetermined and unknown, respectively.
