DECLARATION OF RODIGO IBARRA SILVA l, Rodrigo lbarra Silva, declare and certify the following: 1. I am the nephew of Honest Silva lbarra. make this statement based on my personal knowledge and I am competent to testify to the things established here. 2. My uncle and were working at Sarbanand Farms, LLC this summer under H- 2A visas. Sarbanand Farms is located at 4625 Rock Road in Sumas, Whatcom County, Washington. We were treated well at all times and Sarbanand Farms is a good employer. 3. About 12: 15_, PM, Wednesday, August 2, 2017, honestly told me that he had a severe headache. I knew he was diabetic and that he was not taking his medication because it was over. Honestly I arrive at the of?ce and speak personally with SR. Nidia about your health. 80 I worried and took my uncle to the company office. Because my uncle told me the day before that he had ?nished with his diabetes medications. To my knowledge my uncle did not tell anyone anything, I accept myself personally. I spoke with the manager of the farm, Javier Sampedro. Mr. Sampedro looked at my uncle and told me that he should go to the hospital immediately so that he could be checked by a doctor. Another employee, Maria Barragan was going to take my uncle to the doctor's office, but when they were preparing to leave with my uncle in the front seat, Mrs. Bararra could see that she was sweating and so she decided to call 911. 4. After that call, the ambulance arrived in five minutes in front of the main of?ce. The paramedics were working and testing my uncle for probably 15 minutes until they decided that my uncle's high sugar and low blood pressure were too inconsistent. So they rushed to my uncle at the hospital. 5. Mrs. Barragan took me to the hospital where she stayed with my uncle and me until 6:00 pm. At the hospital, they did some checking and determined that my uncle had serious complications, so he was admitted as a patient in the I thought it was stable during the night. 6. Early the next morning, on Thursday, August 3, 2017, Mr. Sampedro came to the hospital to check on my uncle and me. We talked to Dr. John Jefferson. With Mr. Sampredo translating for me, Dr. John Jefferson said that the big problem with Honest was that he is diabetic and when his body does not produce insulin he can not have energy and he can not swallow his own saliva. My uncle had a swollen brain and had a brain infection. That's why he had a big headache. 7. Shortly after 6:10 am, my uncle suffered a cardiac arrest. He was stabilized, but for a short time Dr. Jefferson told Mr. Sampedro and me that he was trying to move my uncle to Harborview Hospital in Seattle because they have better equipment and he will need surgery. 8. Later, on _4th of August, 2017, my uncle was transferred to Harborview Medical Center in Seattle, WA. Company representatives took me to Seattle to be with my uncle. They also contacted the uncle's wife, who is my Aunt Brenda. 9. Although the doctor worked hard to save my uncle's life, he died on August 6, 2017. My understanding is that he died due to complications of his diabetes and not as a result of any working conditions at Sarbanand Farms. 10. I think Sarbanand Farms did everything it could to save my uncle's life. I am grateful for everything they did. 11. I go home to be with my family during this time of pain. Sarbanand Farms will help with my trip home. They are also working with my Aunt Brenda to help transport my uncle's body to Mexico and make funeral arrangements. Sarbanand Farms assures me and undertakes to transfer and cover all the costs of my honest Uncle's body. 12. I would like to return to the United States to work next summer for Sarbanand Farms. Honestly he undenlvent three tests in his head and his brain did not react. I declare under penalty of perjury that the foregoing is true and correct.
