Upon entering the detainee's cell with the guard force, the corpsman found the detainee completely unresponsive and without pulse or respirations. I he corpsman then called for a RN to come from Camp 6 to support and initiated ELS. The RN responded immediately and continued BI.S and applied an AED for potential defibrilator therapy. TheAED showed that no shock was advised on multiple occasions. Upon determining that the detainee would need definitive ACLS intervention, the corpsman and RN on scene called for the 2" duty RN to respond along with an ambulance and qualified driver (corpsman) for medical evacuato i n. Once the 2"^ RN was cae l d toe l ave Camp 6 and respond to Camp 5. the Jon i t Operato i ns Center (JOC) independently notified the SMO of the emergency situation who then contacted Camp 6 for further details. However, as ail responding personnel had already left for Camp 5. the remaining corpsman had no details available. The detainee was ragidly moved to the ambulance and transported wtih a corpsman and RN actvi eyl provd in i g BLS wtiJl Iuargo i rce members tor secuntv.h i e [h/tvr ambulance arrived at the Detention Hospital which has KN and corpsman on .staff per shift. The camp and hospital RNs both quickly decided that they did not have the ncccssarv' support to initiate full ACI.S as there was no provider immediately available at the hospital and thus detoured to the Naval Hospita^r further resu.scitation. The detainee arrived at the Naval Hospital within approxm i ateyl^mn i utes ofn i tialcode response and the duty NavalHo.sptialEmergency Room provider initiated further ACI.S respon.se but ultimately the detainee was pronounced dead a short t i m e l a t e r. Discussion: A ^^^^^vas appropriately called by the guard force upon determining that the detainee could no longer be .seen definitively breathing. Once it was verified that thedclaine ^^ definitively had no pulse or respirations, the situation should have been upgraded to which would have appropriately precipitated the recall of emergency medical support and triggered notification of the duty provider for potential ACLS response. There was inadequate .space in Camp .s to initiate full high quality ACLS especialy when weiglied against the delay of provider response to reach the camps. I'he detainee was transported to the Detention Hospital in accordance with the JMCi SOP but there is no .standing duty provider at all times which limits immediate upgrades in emergency response. Additionally, not all clinic or hospital RNs are ACLS certified nor is there an immediate recall system or rotating duty assignment for those who are. limiting RN-initiatcd ACLS response. In fact the Detention Hospital RN billets arc routinely staffed with Behavioral Health RNs. even less likely to be trained in ACLS response, fhese factors make the use of the Detention Hospital as the immediate evacuation facility potentially hazardous for llill detainee resuscitation. ISNIS6 was imialy very picky about his morning meds - if he was dxmt to enter prayer or was sleepy, he would ask me to leave his m«is in the spladibox and take them later. 1 did this becaum normally, if you did not leave the meds fbr ISNlSd in his spiashbox. he would get really upset That morning around 0430.1SNI56 seemed sleepy to me but I remember he looked me in the eye and waved his hand in the air away and said "later. when he saw I was thc» to give him his moming meds.
