IMPORTANT INFORMATION REGARDING the Consumer Managed
Personal Attendant Services Program

You are getting this notice because your current Consumer Managed Personal Attendant
Services provider, Outreach Health Services, will no longer provide Consumer Managed
Personal Attendant Services after September 30, 2017.

This notice is to infonn you attempts are being made to ?nd another contractor to provide
services starting October 1, 2017. However, if another contractor is not willing to provide the
service you will no longer receive Consumer Managed Personal Attendant Services after
September 30, 2017.

If the state is unable to ?nd another contractor to provide Consumer Managed Personal
Attendant Services, you will be contacted by Regional Community Services staff so they may
complete an assessment for personal attendant services. They will make every attempt to
transfer you to another program if you are eligible to receive Personal Attendant Services under
another program.
