hospitals and surgery centers do not participate in the
PHCS-Practitioner Only network:
The PHCS-Practitioner Only network is administered
by the following organizations:
 Professional Benefits Administrator (“PBA”)
 Group & Pension Administrators, Inc. (“GPA”)
 Benefits Administrative Systems (“BAS”)
Since Community Health Network is not a “Network
Provider” for your plan, you may be held responsible
for up to 100 percent of the billed charges for services
provided by our hospitals and surgery centers.
