Dear President Burger,

On Saturday, October 30, 2017, I called you and tendered my resignation as Commissioner for
the Oklahoma State Department of Health, effective date to be determined. On the following
day, i spoke with Governor Mary Fallin, and also tendered my resignation as Oklahoma Cabinet
Secretary of Health and Human Services. I would request that my resignation be accepted by
the Oklahoma State Board of Health effective immediately.

it has truly been an honor to serve the Board, the Oklahoma State Department of Health
(OSDH) and the people of the great state of Oklahoma. I know the OSDH, which is comprised
of individuals who are incredibly mission driven and committed to improving the health of the
state, will continue to build on the significant improvements we have seen in the health of our
state over the last several years.
