Dear Matt:
Dr. Konopka requests that the Board extend the time to close her practice until November 30,
2107. She requests an extension to that time to allow her to transfer patients to other

physicians, copy records, and handle other administrative matters.

Dr. Konopka realizes she will be required to cease the practice of medicine on October 13,
201 7.

Please let me know if the Board can accommodate Dr. Konopka in what seems to be a
reasonable request.
