(E001515) Date/ Time: 04/14/2016 10:27:48, Thursday Supervisor: (0) Supervisor Review Date Time: NOTREVIEWED Contact: Reference: Follow Up Narrative On 4/12f16, immediately after morning line-up, Detective KD Hendrix responded to Goodyear Tire and Rubber Plant with Detective Thornton and Captain MV Carter for an industrial accident/death. Once on the scene, detectives were lead to Division A, Banbury Skimmer #5 in the back basement area of the plant by ERB member Tim Holman. ERB members Roger Rigney, Ronnie Setliff, Bryan Shore and other unidenti?ed members (emergency response team) were there, CSSU Willard and Cannaday and Of?cer Hat?eld were on scene, along with personnel from HR. Due to having toured the plant on prior occasions, Hendrix was vaguely aware of the purpose of the skimmer machine. The mixture of humidity and oil made the ?oors, steps, rails, etc. slick or tacky feeling, depending on where a person was walking or standing. At the time of arrival, the body of a male, identified by coworkers as that of Charles Gregory "Greg" Cooper (aka Skinny) was face down on the ?oor of the pit next to the opening of the steam/water pit. Cooper?s body was Covered in oil. A rope Was tied around his ankle and a pulley With a hook and chain was hanging above the pit. It was explained that the hook was used to help pull Cooper from the water. The cover the side, as it had to be removed to retrieve Cooper?s body. Hendrix was advised by Hat?eld that TOD was called at 7:153m by David Rogers. A?er viewing the scene and Cooper" body had been removed, Thornton, Hendrix, CSSUconference room in the of?ces of the plant. Bob Eastwood had been working as Acting Area Manager until 1:00am on 4f12/1 6, at which time Samuel Lee Richardson reported to work. According to Eastwood, at approximately 8pm when he was doing a walk-thru, he noticed the rope/skimmer tube was broken, ordered one from the store room and put in a work order for Cooper to make the repair. Eastwood explained that a broken skimmer tube was a non-emergency, routine, priority 2 work order. Priorty 2 means, get to it when you can. Eastwood advised that the pump has been broken on that machine for months. The part had been ordered but had not yet arrived. The broken pump had been removed and was laying to the side of the pit, making the opening to the water pit larger. Hendrix had made note of the large pipe/pump piece while viewing the scene of the accident. Cooper was a member of the maintenance crew at the plant and had been working there since 1998. Eastwood stated that someone else had told him that Cooper "did not look good" earlier in the evening, but Eastwood said he did not notice anything different about him during the maintenance meeting when assignments are given out. Eastwood mentioned more than one time that Cooper worked alone most of the time, but more than one employee, all of which wish to remain anonymous, has mentioned to Hendrix that since the industrial accident in the Banbury in 2007 or 2008, where an employee was severely burned and later died, it was policy that all maintenance workers working in the Banbury area of the plant work in pairs. They advised Hendrix that at no time were maintenance workers to work in that area alone, as it is one of the most dangerous areas of the plant.
