SAFETY NARRATIVE - . 1049252 I covenant: INFORMATION: On 03/26/20 1 5, CSHO (7)(C)arrtved at the jobsite after receiving a referral from the employer, (referral number 970724). The presented credentials to Safety Manager, Mr. Charles Skaggs, and Union Rep, Mr. Rick Cli?on, and conducted an opening conference. The referral was about an employee gelling injured while cutting the rubber off from the conveyor tail roll in the Mixing Department area. The employee Fractured the left arm, left shoulder, four ribs, and broke his collar bone. The accident took place at the Laydown #8 location of the Mixing department, around 8:00 am. The company is located in Gadsden, AL, and was established on 12/13/1928. Goodyear specializes in the manufacturing of commercial automobile tires with a head size of 14 inches to 17 inches. This is a union facility - withlb) (4lernployees at the Gadsden location and over (4) employees corporate wide. The company works 24 hours, 7 days a week with 4 different rotating crews. Referral itcm(s): I. Employer stated his company had an employee hospitalization. Employer stated an etnpioyee working in the mixing department sustain a serious injury to his left arm/shoulder. The employee was interviewed and states he saw rubber between the takeaway belt and end roll making the belt pull to one side. The employee beganjogging the belt trying to get the rubber out and got his hand caught between the roll and belt which pulled him in up to his shoulder. The employee was cared for by on site and transported to(b) (7) (C) Inspection finding(s): 1. During the inspection, the Compliance Of?cer veri?ed that on 3/22/20 S, at approximately 8:00 am, an employee fell 29 inches onto the conveyor belt of the #8 Laydown mill machine at the Mixing department. The company LOTO program indicated that the injured employee was not an authorized LOTO employee. The employees conduct work at the edge of the platform at least 5 times on the average per work day. The machine operators get close to the edge of the platform to receive rubber coming from the tail roll machine located above the conveyor belt. The conveyor belt is 29 inches below the working platform. The employee fractured the left arm, left shoulder, four ribs, and broke his collar bone. During the investigation, the CSHO determined that the platform where the injured employee was working from needed to be - guarded with standard railings. It was also determined that the ?oor of the work station platform had oil that leaked from the adjacent machinery.
