The following medical conditions were removed from UR 601-210 M-5b which were part of the "never waiver/suspended? status. a. Self-mutilation b. Non-isolated mood disorders (to include depression and bipolar (does not include e. Drug and alcohol abusefdependence as reflected in the Diagnostic Statistical Manual (DSM V) that is not in sustained remission.? Does not include court- ordered evaluations as part of a sentencing. 3. Implementation: To properly recommend a waiver for an applicant with any of these conditions, the following documents are required: a. Photos of suspected self-mutilation (to be taken by recruiter) i. Photos must be clearly labeled with the area of body outside area of right forearm or inside portion of left wrist), applicant?s name and PRID, and the date/time the photo was taken. Do not take/provide any photographs of intimate areas. b. Detailed applicant statement. 0. Supporting medical records as needed. 4. in many cases, a consult will be accomplished through USMEPCOM, but civilian clearance may be'suf?cient. 5.
