My instructions were to conduct an origin and cause investigation. The instructions were updated on June 15, 2000 to include the submission of debris samples collected for forensic analysis. Approval was also obtained to retain the services of Forensic Technician, Larry Miller, to assist in the examination of electrical and mechanical causal factors. Mr. Miller will ?le a supplemental report concerning his ?ndings. DETERMINATION OF ORIGIN AND CAUSE This ?re has been determined to be arson. The ?re originated in the east side of the structure, in a hallway between the church administrative of?ces. The cause of the ?re was determined to be an ignitable liquid poured down the center of the hallway, then intentionally set on ?re. Information and factors available to establish origin: 1. Examination of all electrical and mechanical components of the structure FIRE SCENE EXAMINATION On June 13, 2000, I met at the scene with Mr. Wayne Barnes and the Federal Bureau of Alcohol, Tobacco and Fire Arms' Senior Special Agent, Mike Jaraczeski, who were in charge of the scene examination. Detective Jeff Richardson, Detective Steve Hall and Detective Bob Smith, with the Jefferson County Police Department were also present and requested the assistance of BATF. Agent Jaraczeski informed me I would not be able to conduct the scene examination while BATF was conducting their scene examination. However, Agent araczeski agreed to allow me to come into the perimeter to photograph the exterior fire scene prior to the structure being altered by heavy construction equipment. Lt. Steve Weddington, Arson Canine Handler with the Lexington City Fire Department, would be on the scene Wednesday. No evidence or sample collections would be collected until the arson detection canine searched the scene. It was further agreed that I would be allowed into the perimeter at the end of each day to again photograph the scene alterations from the exterior of the structure. All electrical and mechanical components would be set aside for examination. Detective Hall informed me that no scene work was being done at this time. Heavy construction equipment was en route to the scene to assist in the removal of the collapsed second ?oor. Detective Hall stated that, at the present time, interviews were being conducted by the Jefferson County Police Department and the Bureau of Alcohol, Tobacco, and Fire Arms concerning the first responding fire units and members of the church. A neighborhood canvass was also in progress. Detective Hall stated that the backdoor was found unlocked by the first responding fire units. OH01-01917 4 July 7, 2000 INSURED: Heart of Fire Church Outreach Ministries I was requested to wait until the interviews with the ?re department were complete before they were asked to give me a statement. Upon completion of the meeting with the Police of?cials I was allowed to photograph the exterior of the structure. The involved structure was a two-story wood-framed church. The structure consisted of a sanctuary on the north side and a new, two-story addition on the south side. The new addition contained a child day care and administrative of?ces of the church. The structure was housed under a composition shingle roof with a concrete slab foundation. The structure was serviced by both electrical and natural gas utilities. The electrical service entered the structure on the west side and natural gas entered the structure on the northeast side. An examination of the utilities located on the outside of the structure revealed no ?re involvement. The roof and second story of the structure suffered signi?cant ?re damage and collapsed into the ?rst ?oor. The roof over the sanctuary was a vaulted ceiling and was consumed during the ?re. The heaviest ?re damage visible from the exterior was located on the east side in the area of the church of?ces and adjoining hallway. On June 14, 2000, I returned to the ?re scene and met with Senior Special Agent Jaraczeski, Detective Richardson and Detective Hall. Agent Jaraczeski informed me the main effort today would be to remove hazardous walls that may collapse into the structure injuring investigators and to clear debris away from a hallway located inside the rear entrance to the church of?ces. Lt. Weddington?s canine would then be allowed to search for evidence of an ignitable liquid. I would not be allowed to take any photographs inside the perimeter until the structurally damaged walls were torn down and Lt. Weddington completed his search with the canine. Agent Jaraczeski requested Forensic Technician, Larry Miller, respond to the scene to assist in the examination and elimination of electrical and mechanical components as a causal factor. Mr. Miller responded and examined the circuit breaker boxes, service drop and meter bases for the structure. Mr. Miller will address the results of his examination in a supplemental report. On completion of Lt. Weddington?s search of the scene with his canine, and collection of samples, Agent Jaraczeski informed me I would be able to photograph the interior scene alterations. While inside the structure near the rear hallway, I detected the odor of gasoline. Agent Jaraczeski stated that if I went through the decontamination station, I would be allowed to enter the hallway, photograph it, and collect samples for forensic analysis. After photographing the hallway, I collected one sample of carpeting and padding located in the northwest corner of the hallway and a sample of vinyl flooring in the center of the hallway. The hallway floor was then washed off with fresh water to remove any ash and debris. This exposed a burn pattern down the center of the hallway at floor level. This - a 5 July 7, 2000 INSURED: Heart of Fire Church Outreach Ministries type of burn pattern re?ects the presence of ?re burning at ?oor level in an area that should not have displayed this type of damage with normal ?re spread. The ?re damage to the hallway floor was consistent with the presence of an ignitable liquid being present on the ?oor that arti?cially spread the ?re into this area. Lt. Weddington stated that his arson detection canine indicated on several locations within the hallway. BATF collected samples at each location his arson detection dog indicated. On June 15, 2000, I again met with Agent Jaraczeski, Detective Hall and Detective Richardson. I was informed that search warrants were being prepared for the business of?ces of the church and a vehicle parked on the church lot. Agent Jaraczeski requested I provide him with the name of the insurance company and policy limits concerning the church. Mr. Wayne Barnes was contacted and advised he would provide BATF with the insurance information requested. INVESTIGATION The Reverend Danny Johnson has retained a private attorney and will not be interviewed. On June 15, 2000, Special Agent?in?charge Mike Jaraczeski stated he would not be able to comment on the investigation. The Bureau of Alcohol, Tobacco and Firearms would only provide a copy of the investigation to the Jefferson County Police Department. On June 15, 2000, I Spoke with Detective Hall and Detective Richardson. Detective Hall stated that the ?re was determined to have originated on the east side of the church in the area of the hallway leading into the church of?ces. It was his determination that the ?re was arson. During the ash and debris removal in the rear area of the church, Detectives Hall and Richardson detected the odor of gasoline. The debris was removed from the hallway located inside the rear of the church between the of?ces. Detective Richardson stated he determined an ignitable liquid had been poured onto the floor of the hallway. Detective Richardson stated that during a neighborhood canvass, a witness reported seeing a large gray vehicle pull out from behind the church with its headlights out and pulled onto Bardstown Road southbound at a high rate of speed. Detective Hall would not provide the identi?cation of the witness, only adding that the witness was not a member of the church. On June 15, 2000, I spoke with Lt. Steve Weddington, of the Lexington City Fire Department. Lt. Weddington stated that he and his arson detection dog are members of the Bureau of Alcohol, Tobacco and Firearms National Response Team. 6 July 7, 2000 INSURED: Heart of Fire Church Outreach Ministries His dog indicated in several areas in the hallway at the rear of the church. Samples were collected for forensic analysis. Special Investigator David Green has interviewed the ?rst responding ?re of?cials and will ?le a supplemental report. MMENT A verbal report was made to Mr. Wayne Barnes on a daily basis. Mr. Barnes was also advised that Special Investigator David Green would complete the scene examination and interview the ?rst responding ?re of?cials. Mr. Bames instructed me to submit the two debris samples collected for forensic examination. As instructed, I have packaged and mailed the debris samples and prepared this report.
