I IOKLAHOMA STATE DEPARTMENT OF HEALTH - CREATING A STATE OF HEALTH MODIFICATION to the CONTRACT between THE OKLAHOMA STATE DEPARTMENT OF HEALTH And THE OKLAHOMA TOBACCO SETTLEMENT ENDOWMENT TRUST (TSET) It is agreed between the parties hereto that the contract entered into May 16, 2011 between the Oklahoma State Department of Health, by virtue of the authority vested in it by Title 74, 0.8. Section 581 and The Oklahoma Tobacco Settlement Endowment Trust (TSET), purchase order number 3409015559, is hereby modified as follows: The contract will be increased from $3,500,000.00 to $8,500.000.00, (an increase of $5,000,000.00) for the period of 5/16/2011 thru 5/15/2012 The Contract will be modified to add the following: Under Purpose ADD: Develop a grant program for school districts to enable implementation of effective school based wellness policies and interventions to improve student, teacher and staff health; enhance family involvement in school and student wellness; and reinforce other school based programs focused on health and wellness. Under TSET Duties ADD: 5. Utilizing the Centers for Disease Control and Prevention (CDC) Coordinated School Health Model, and in partnership with the OSDH, develop a policy?based grant program aimed at improving student and staff wellness 6. Promote and advertise grant program to school districts and school administrators statewide 7. Assist in the development of evaluation criteria and make contractual arrangements for program evaluation 8. Funding for school districts will be used exclusively for the implementation of wellness policy or wellness activities outlined in the CDC Coordinated School Health Model and should be targeted to low performing, limited access school sites 9. Program should be available no later than the beginning of the 2012 2013 school year Under OSDH Duties ADD: 5. Coordinate with TSET on the development of school grant criteria 6. Promote the Certified Healthy School Grant Program locally through Turning Point and County Health Departments 7. Provide education and technical assistance on effective school-based wellness policy to school district administrators and staff 8. Assist school district personnel in the collection of data for program evaluation Under Period ADD: Effective upon approval of amendment, contract period will coincide with existing contract timeline (May 16 through?May15 each period) Under Funding ADD: $5,000,000.00 will be available over the period of the full contract including all contract renewals. In order to ensure funding availability to local school districts the OSDH will make resources available to TSET in the first year upon receipt of an invoice. Within 30 days of the conclusion of each contract period, and upon contract renewal, TSET will provide a reconciliation of all amounts expended and remaining balances on this contract.
