The owner needs to continue their efforts to remove the woody vegetation along the shoulders of the crest, along the upstream slope, within 5?feet ofthe spillway walls, and along the toe area. The owner needs to continue to investigate and repair the spillway drain system that appears to be causing the reported seepage originating in the concrete V-ditch near Sta. 7+80 of the spillway. The most recent video inspection did not locate the cause of the problem, and another investigation is currently being planned. This work is considered maintenance and will not require an application. The most recent instrumentation submittal dated November 11, 2014 included most of the additional items that were requested in our previous inspection reports and correspondence. The following items need to be included in the next instrumentation submittal: 1. Short?term plots spanning one to five years for the open well piezometers. 2. Plan and cross?sectional views of the open well piezometers. which include the sensing zone. If available, provide as-built drawings ofthe open well piezometers for inclusion into the Division's files. The instrumentation submittal for 2015 is overdue, and submittals need to be submitted on a consistent and annual basis. An instrumentation submittal with data through the calendar year of 2015 needs to be submitted by July 1, 2016. Conclusions From the known information and visual inSpection, the dam, reservoir, and the appurtenances are judged safe for continued use. Observations and Comments Da_m The asphalt paved crest, which is Lopez Drive and maintained by the County, appeared uniform and well aligned. The visible portion of the upstream slope was inspected from the crest and the spillway approach, and it appeared in stable and in satisfactory condition. The slope was inspected from the crest, access road, groins, and toe, and appeared uniform, stable and in satisfactory condition. The concrete V?ditches that line both the left and right groins appeared in satisfactory condition and were adequately clear of debris. The groins and toe area appeared normal and the abutments stable. Vegetation control of bushes and other woody vegetation was overall good. I observed sporadic woody vegetation along the both shoulders ofthe crest, along the upstream slope, and locations throughout the toe area. In particular, the oak tree that has become established located on the shoulder of the crest, near the left abutment, needs to be removed. Observed rodent activity was minimal, but control efforts need to be continued. The approach, ogee weir, control channel, and exit were clear of concerning debris. walked the entire length of the spillway channel and along the left spillway wall. The concrete comprising of the ogee weir, the invert, and the walls appeared in satisfactory condition, having no sign of instability or distress. The approximate 2?inch offset at the top of the right panel wall joint, located two panels from the bridge, appeared unchanged as compared with previous inspections. Other than the above mentioned wall panel joint, the invert and wall panei joints were welt aligned and the spillway appeared in stable and good condition. All the weep holes were clear and dry. For upstream control, this dam has seven 20-inch butterfly valves and a 36-inch butterfly valve installed in a sloping intake structure. The 36-inch butter?y valve is located at the bottom of the structure and isolates the sloping structure to the entrance of the outlet pipe. Reportedly, the 36- inch valve is never operated, and since it is redundant, it is not needed for dam safety. The six upper upstream butterfly valves were fully cycled, and the valve located at the lowest elevation was not cycled due to sediment concerns. The operation of the upper six inlet valves went without incident. The owner is'considering a dive inspection to determine the sediment conditions near the lowest inlet valve. Due to the lower water level, I was able to inspect the exterior portions of the concrete intake structure to Valve The concrete comprising the structure appeared in satisfactory condition, having no sign of instability or distress. control for the low-level outlet consists of a 42-inch knife gate blowoff valve, and-the blowoff valve was fully cycled. The operation went smoothly and no problems were encountered. The concrete energy dissipating structure for the exit to the knife gate blowoff appeared in good condition. Although the reservoir?s water level was unseasonably low, I observed no seepage or signs of seepage on the face, along the groins, or toe area. I paid particular attention to the area between the left spillway channel wall and the right groin. On February 9, 2012, the owner observed seepage originating from the left spillway V-ditch near spillway Sta. 7+80. The seepage was approximated at 1/1 (reservoir water was at El. 516) and is believed to have originated from the spillway underdrain system. The spillway wall underdrains typically start flowing when the reservoir level rises above El. 503. The reservoir water elevation was at El. 467.91 during this inspection and this area was dry. As discussed in the previous inspection report, a video inspection was performed approximately two years ago, and the camera was too large to travel through the 8-inch diameter vitrified ciay pipe (open joints) section along the left spillway wall. The video investigation was successful in traversing the 10-inch diameter section of the pipe, upstream of spillway Sta. 8+30, but could not enter the 8-inch diameter clay pipe located at Sta. 7+10. The portion of the pipe that was inspected did not indicate any problems. According to Mr. Boyce, the owner is preparing to run a smaller camera through the underdrain at a later date. This work is considered maintenance and an application will not be required. The seepage from the three right abutment drains and the three left abutment drains were all dry. The dry conditions for the spillway subdrains, and the right and left abutment drains, are normal for the iow reservoir water level during this inspection. Instrumentation at this dam consists of 29 vibrating wire piezometers, nine open well piezometers, six abutment drains, three spillway drains, eleven survey monuments located on the crest, ten survey piliars located on the bench, and six spillway pins. The latest instrumentation submittal was dated November 11, 2014 and the reporting period covered through April 2014 This submittal was previously reviewed in the May 19,2015 inspection report, and the data and review indicated the dam was performing adequately.
