The clinic note from that visit is included With this letter in this letter you noted that previous reports "do not medically explain how your accepted conditions have worsened such that you stopped work on 03/13/2016" nor do they "provide an explanation of how you are disabled from performing the duties of your employment as a result of your work injury and accepted conditions Mr. Albert has an accepted condition of irritant induced irritable larynx and vocal cord dysfunction as a result of his workplace exposures He also has irritant-- induced asthma (reactive aiMays dysfunction - RADS) also as a result of his workplace exposures and confirmed by a methacholine challenge test on 7/20/2016. This latter diagnosis should be added as a diagnosis on his claim. He stopped work on 08/13/21016 because he was again exposed, against medical recommendations, to respiratory irritants at work. This resulted in a temporary aggravation at his now longstanding respiratory conditions. He has not worked since apparently due to the inability to accommodate his needed work restrictions. Mr. Albert is not disabled from performing the duties of his employment as long as his exposures to gases. dusts, fumes and vapors are minimized. While this was achieved previously, because of changes to his work environment. this is not the case any longer I hope this has helped to clarify the concerns in your letter Please get back to me if can be of further assistance. Sincerely, Electronically signed by Sverre Vedal, MD CC: OWCF, US Department of Labor OCCUPATIONAL AND MEDICINE 325NmthAve Seattle,WA 93104 zoo-74479372 Fex2057744~8527 a)
