To Whom It May Concern:

My name is Rebecca Fraynt, and I saw Kevin Albert for an initial therapy
appointment on 10/31201 4. During this visit, I diagnosed Kevin with:

(296.22) Major depressive disorder, single episode, moderate

This letter is to request that major depressive disorder be added as one of
Kevin's diagnoses covered under his worker?s compensation claim. It is my
opinion that Kevin's current depressive disorder was caused by his industrial
injury. Kevin reports no history of depression prior to his industrial injury. He is
reporting significant oCcupational and health stressors directly related to his
injury that are likely strong contributors to his depressive PHQ-Q
scores are currently in the moderate range, confirming a major depression

diagnosis.

The following is aproposed treatment plan for Kevin's 

Frequency: Every 2-3 weeks
Duration: 3?6 mOnths
of recommended treatment plan.

Goals are to help reduce depressive improve sleep. We will use
CBT tools to help patient improve interpersonal effectiveness skills so he can?
reduce his own caregiver burden in his family and get support from appropriate
family members and friends. We will also work to improvethese skills in the
work setting, so he can feel less stress in his supervisory position. We will
discuss behavioral sleep hygiene techniques and behavioral activation as well.
Treatment progress will be tracked by looking for significant reductions in PHQ-Q
scores (5 points or more). Prognosis is good, as patient has good insight in his
problems, has history of strong coping, and does not have previous history of

depression. ..
