Declaration of Sylvester Jackson
Pursuant to 28 U.S.C § 1746, I certify under penalty of perjury that the following
statements are true and correct:
1.

My name is Sylvester Jackson. I am 50 years old. I was convicted in

Wisconsin in 2008 of a sex-related crime. As a result of my conviction, I am classified as a
“Designated Offender” under Chapter 106-51 of the Milwaukee Code of Ordinances.
2.

At the time I committed my offense, I lived in Milwaukee, Wisconsin.

3.

I currently live in Milwaukee and am homeless because I cannot find any

compliant housing.
4.

I don’t want to be homeless, and I have looked for compliant housing on and

off for years, but all of my efforts have been futile.
5.

I usually sleep in my broken-down car, which doesn’t start due to a defective

alternator.
6.

I have numerous serious health problems, including diabetes, sleep apnea and

asthma. I must wear a c-pack breathing machine at night to breathe properly. I fear that being
homeless puts my life at risk for two reasons: (i) I am unable to plug in my c-pack machine at
night; and (ii) I do not have ready access to refrigerated food to allow me to eat properly,
which, given my diabetes, is a serious problem for me.
